3. The table on page 3 -9 is not meant to indicate that the water well permit was <br />terminated. The date of 3/17/82 reflects the application or approval date. Text has been <br />added on page 3 -9 to clarify the table contents. <br />4. Page 3 -9 has been revised to reflect the current address for the La Plata County <br />Clerk's Office. <br />5. Pages 3 -10 and 3 -11 have been revised to reflect the currently approved Public Notice. <br />2 05.3- Operation Plan- Permit Area (Revised pages 5-1 through 5 -6)) <br />1. The designs for the original sediment control system and revisions are contained in <br />Appendices 5 -2, 5 -9, 5 -10, and Map 5 -10 (referenced in the Abridged Permit Document, <br />updated after TR 12). The text in the current Abridged Permit Document has been <br />revised to reflect the above. <br />2. Page 5 -3 text has been changed to reflect that the approved designs for the Carbon <br />Junction Channel are found in the prior permit application document. As the channel has <br />been constructed and Phase II released, we questioned the necessity for retaining the <br />designs in the current Abridged Permit Document, and deleted the design text from the <br />RN04 version. <br />2.05.4 — Reclamation Plan (Revised pages 5 -15 through 5 -16) <br />1. Let me recap information regarding the following wells drilled within the current or <br />former Carbon Junction Mine permit area. As an aside, documentation in the original <br />and subsequent Carbon Junction Mine permit application and permit documents indicates <br />the required information on the wells was incorporated into the CDRMS permit <br />application and filed with the state Division of Water Resources (this includes drillers <br />logs and well drilling applications, and where appropriate, permits for use). Our records <br />contradict those of Mr. Foy at the state Division of Water Resources with regard to his <br />letter of April 11, 2013. Division of Water Resource comments will be addressed under <br />separate cover. <br />Well PWI (AKA 82-4)- Information regarding the drilling and completion of this well (as <br />documented by applications to DWR and drillers reports) is contained in the original <br />permit application documents for the Carbon Junction Mine. This well was permitted <br />through the State Engineer's Office as an industrial water supply well. Subsequently, <br />that permit expired. Currently, the well is still in place and has been sealed with a welded <br />steel cap. <br />Well B10- Information regarding the drilling and completion of this well (as documented <br />by applications to DWR and drillers reports) is contained in the original permit <br />application documents for the Carbon Junction Mine. The well has been sealed in <br />accordance with applicable Division regulations, and written and photographic evidence <br />was submitted to the Division under separate cover in 2012. <br />Well B17- Information regarding the drilling and completion of this well (as documented <br />by applications to DWR and drillers reports) is contained in the original permit <br />application documents for the Carbon Junction Mine. The well has been sealed in <br />accordance with applicable Division regulations, and written and photographic evidence <br />was submitted to the Division under separate cover in 2011. <br />
