June 27,2013 C-1981-012/New Elk Mine RDZ <br /> The following culverts were inspected(in chronological order): the culverts southeast of Pond 007A,C48, <br /> C59,C17,C17A,C12A,C73,C16A,C13,C30 and nearby culvert to east, C-A,C45, C39,C40, C81, <br /> C41, C3, and C37. Most of the culverts meet the PAP specifications and are functional,requiring no <br /> immediate maintenance. Some repair and maintenance is required: <br /> • One of the culverts southeast of Pond 007A requires cleaning. <br /> • C17 requires minor vegetation removal at the lower end. <br /> • C73 is full of sediment, and ditch D15 needs to be deeper. <br /> • C16A needs to be replaced. <br /> • Culvert C30 has been replaced and a new culvert to the east has been added(under the road to <br /> State Highway 12). <br /> • C-A and C81 require minor cleaning. <br /> • Replacement of C41 is not complete. Also,there is a gully above the downstream end of the <br /> culvert,which should be monitored to make sure it does not head cut too close to the <br /> conveyor. <br /> • Part of C3 has been constructed (the upstream portion) and part of it(the lower portion) <br /> still needs to be replaced. <br /> • C37 requires cleaning, mostly at the upstream end. <br /> The following ditches were inspected(in chronological order): D26,D24A,D23,D22,D16,D15,DI7A, <br /> D1713,D10,D12,D19,D14,D56, and the ditch along the west side of the Refuse Disposal Area(RDA). <br /> The following was observed regarding the ditches: <br /> • Ditch D26 has been cleaned,and the water pipe that was causing an obstruction has been <br /> removed. <br /> • Water was flowing in ditch D23, and was very clear. The source of this,per Mr. Thompson,is a <br /> sump in the pedestrian tunnel. <br /> • The upper portion of D22 is smaller than the specification in the PAP. However,this does not <br /> appear to be a problem as it is below the confluence with ditch D32 and drains a small area. It <br /> should be monitored in the future. <br /> • Ditch D16 has been cleaned. <br /> • Ditch D15 needs to be cleaned. <br /> • Ditch D17B requires more definition near the fan. <br /> • The outfall for ditch D I O has been constructed. Many of the rocks in the riprap apron are greater <br /> than three feet in diameter. NECC will hydromulch the banks in the ditch above this outfall <br /> during the summer of 2013,per Mr.Thompson. <br /> • Based on a comparison to the specification in the PAP, ditch D12 is too shallow and narrow <br /> for much of its length, and NECC needs to address this problem. <br /> • D19 has been cleaned and hydromulched. <br /> • D56 is not well defined, and the direction of flow is unclear(map could be wrong). <br /> Best Management Practices for sediment and erosion control for two SAES were inspected. Near Pond <br /> 007A the silt fence was functioning properly. The silt fence near culvert C30 requires maintenance. <br /> GENERAL MINE PLAN COMPLIANCE: <br /> The mine was generally found to be in compliance with the mine plan. <br /> Number of Partial Inspection this Fiscal Year: 8 <br /> Number of Complete Inspections this Fiscal Year: 4 <br /> Page 3 of 11 <br />
