Southfield C1981014: MT6 12 February 19, 2013 Section III - Status of Stipulations The stipulation history for the Southfield Mine was reviewed as part of the midterm review. Review included investigation of the stipulation response file, revisions to the permit, and the findings document for RN -05. Stipulations are a binding part of the permit and previously assigned stipulation numbers are not to be re -used. Any stipulations associated with this permit and issued over the life of this operation which are not discussed in this midterm review have been complied with or terminated. As of the time of this midterm permit review, there are no active or un- resolved stipulations to the Southfield mining permit. Section IV - Enforcement Actions There has been one enforcement action taken in the last two and a half years since approval of the last permit renewal (RN -05). Notice of Violation CV- 2013 -002 was issued on January 28, 2013 for failure to maintain a water monitoring well as designed. The Division found the MW -NW was obstructed and water monitoring equipment was unable to access the approved depth of the well as per the approved design. EFCI was provided time to clear the well, but failed to do so. This violation is active at the time of this mid -term review. Section V - Identified Issues Review of the permit application package resulted in the following questions or issues. 1. During the Division's review several discrepancies within text, tables, figures, and maps were identified. Please correct the following: • Figure 1A is not listed in the Table of Contents (TOC). • Figure 21 is listed in the TOC but is not in the report. • Figure 22 is listed in the TOC under a different page than it is in the report. • The title of Figure 23 is different on the figure than it is in the TOC. • The title of Map 9 is different on the map than it is in the TOC. • A discussion of access roads (Section 4.03.2) appears to be missing from the PAP. • Pages 2.05.6 -53a does not flow to the next page (2.05.6 -54) properly. 2. Please provide state well permit information in accordance with Rule 2.03.10. All monitoring wells are required to be permitted with Colorado Division of Water Resources; Sate Engineer's Office. Please provide DWR -SEO permit information for all monitoring wells approved in the permit. 3. Please assure that Table 15 -Well Permits are up to date, and well locations are correctly located on the Mine Area Surface and Groundwater Hydrology map (Map 12). [Rule 2.04.7(4)(a)]
