January 28, 2013 C-1981-012/New Elk Mine LDS <br /> • The "substation" SAE showed no recent disturbance. <br /> • Ditches and culverts were in good order,unless otherwise noted. <br /> • The system of three culverts that make up the clean water culvert C2 remained as they were during the <br /> January 2, 2013 inspection,with a trench bypassing the downstream section(see photo 1). Either the <br /> appropriate sections should be removed,leaving the centre section in place; or the trench should be <br /> filled in and the upstream and downstream culverts should be cleaned out.In either case the permit <br /> should be updated to reflect the physical reality on the ground. <br /> • The outlet to clean water culvert C3 was partially blocked. The culvert should be cleared out. <br /> • Machinery and equipment was encroaching into ditch D5 (see photo 2). This equipment should be <br /> moved. <br /> • The slurry injection SAE showed no recent disturbance. <br /> • Pond 4 was dry. The embankment was sound and both the primary and emergency spillways were in good <br /> order. <br /> • The ditches and culverts around DWDA 1 were clear and functional. <br /> • Water flowing through the clean water ditch which conveys water from the highway culvert west of the <br /> main gate to the Purgatoire River had eroded the gabions near the river(see photo 3). The gabions should <br /> be replaced or restored to ensure that excessive erosion does not occur. <br /> • There was no evidence of recent disturbance at the lower of the two water tank pads; however uphill at the <br /> larger pad the low spots in the berm noted previously had not been built up (see photo 4). The berm <br /> around the SAE should be built up where it is low. <br /> On the north side of the highway: <br /> • The upland diversion ditch above the RDA was clear and in good order. There was no evidence of <br /> breaching. At the time of the inspection a survey of the regraded ditch was in progress. <br /> • There was no sign of recent disturbance at the cell phone tower pad, or the lower pad located immediately <br /> to the north. <br /> • The damaged section of the culvert under the conveyor, C41,had been replaced and the area around it <br /> filled and restored. <br /> • All culverts under the haul road up the west side of the RDA were clear. <br /> • The embankments and spillways of pond 8 were sound. <br /> Between the highway and the Purgatoire River: <br /> • The temporary conveyor that had previously constricted access in this area had been removed.Work was <br /> underway to remove culverts C70 and C71 (see photo 5). This project should be completed as soon as <br /> possible,with the relevant changes made to maps and tables in the response to the mid-term review. <br /> • Work was underway to re-route the flow from the first highway culvert (roughly north of the eastern end <br /> of DWDA 2),and second highway culvert(east of the small gate),to culvert 00. The culvert will be <br /> replaced with a 48"diameter culvert. The same project had led to the removal of culverts C14 and C14A, <br /> with the intention of diverting flow to ditch D19. This project should be completed as soon as possible, <br /> with the relevant changes made to maps and tables in the response to the mid-term review. <br /> Number of Partial Inspection this Fiscal Year: 4 <br /> Number of Complete Inspections this Fiscal Year: 3 <br /> Page 4 of 13 <br />
