ameO <br />1.0 INTRODUCTION <br />Cripple Creek & Victor Gold Mining Company (CC &V) operates the Cresson Project, located in <br />Teller County, Colorado, near the city of Victor. The Cresson Project is a mining and ore <br />processing and beneficiation facility that is composed of surface mines as well as a valley leach <br />facility (VLF); an adsorption, desorption, and recovery (ADR) plant; and associated <br />infrastructure, all of which were designed and constructed to meet or exceed the requirements <br />established by CRS §34 -32 -101 et seq. and regulations promulgated there under by the Mined <br />Land Reclamation Board (MLRB). <br />The work associated with the Cresson Project was performed under specific criteria established <br />in Amendment Nos. 8 and 9 of Permit Number M- 1980 -244 (Permit), as approved by the MLRB. <br />This Report documents the Construction Quality Assurance (CQA) and Construction Quality <br />Control (CQC) services provided by AMEC Environment and Infrastructure (AMEC) from <br />October 10 through October 12, 2011 for the extension of the liner system at the VLF Load Out <br />Bin (LOB). The general location for the LOB Liner Extension is presented on Figure 1. <br />1.1 Project Description <br />The existing Cresson Project was constructed in phases from 1994 through 2011 (Phases I <br />through 5). Phases I and II were permitted in 1994 and constructed during the 1994 through <br />1996 construction seasons. Phase III was permitted in 1999 and construction was initiated the <br />same year, with completion in 2000. Phase IV was permitted in 2000 and construction was <br />completed in 2004. Phase 5 was permitted in 2008 and was completed in 2011. As part of the <br />construction activities, the LOB was moved from its previous location to a new pad located to <br />the north end of the VLF, as shown on Figure 1. The liner system was extended to allow <br />containment of the haul truck traffic at the LOB in October 2011. The composite liner system <br />consisted of a geosynthetic clay liner (GCL) and 80mil LLDPE Single Sided Texture. <br />1.2 Construction Quality Assurance /Quality Control <br />AMEC provided CQA monitoring and testing services, and prepared this report describing the <br />monitoring and test results for the LOB Liner Extension. Figures and supporting CQA monitoring <br />and testing documentation are presented in appendices attached to this report. <br />1.2.1 CQA Monitoring of Earthworks <br />AMEC provided CQA testing and monitoring of earthworks for the LOB Liner Extension. CQA <br />testing and monitoring of earthworks was provided for placed materials in accordance with the <br />AMEC Earthworks CQA Plan.
