The Applicant provided the following table of the projected replacement obligation for the
Hazeltine Reservoir site. The projection assumes that the liner for the reservoir will be fully
completed and tested by August 2013 after which evaporation and other consumptive uses will
cease.

Conditions of Approval
I hereby approve this SWSP in accordance with § 37 -90- 137(11), C.R.S., subject to the
following conditions:
1. This plan is approved with an effective date of October 1, 2012 and shall be valid through
October 31, 2014 unless otherwise revoked or modified. If this plan will not be made
absolute by a water court action by the plan's expiration date, a renewal request must be
submitted to this office with the statutory fee (currently $257 /pit) for each gravel pit no
later than September 15, 2014.
2. Well permits 60313 -F, and 67261 -F has been obtained in accordance with § 37 -90-
137(2) and (11), C.R.S., and remain in effect for Road Runners Rest II pit and Howe pit,
respectively. Should a new well permit be required but then denied for reasons of 600
foot spacing, or any other legitimate reason, approval of this substitute supply plan may
be cancelled.
3. A well permit has not been obtained for the Reclamation Pit No. 1. This pit is a part of
Hazeltine Reservoir which includes the neighboring Road Runners Rest II pit. The
exposed surface area of the Hazeltine Reservoir is within the limits set forth under the Road
Runners Rest II pit well permit. The Hazeltine Reservoir has been lined and will undergo
the liner leak test in WY 2013, after which a well permit will not be required. For this
reason, the Applicant is not currently required to apply for a well permit for Reclamation Pit
No. 1. However, If the reservoir is not approved as a lined reservoir by the end of WY
2013, then a well permit will be required in accordance with § 37 -90- 137(2) and (11),
C.R.S. Should a well permit be denied for reasons of 600 foot spacing, or any other
legitimate reason, approval of this SWSP may be cancelled.
