6.0 GEOMEMBRANE CQA TESTING AND MONITORING <br />ame0 <br />The following sections describe the CQA activities performed by AMEC for the <br />geomembrane installed in the 2011 Phase 5 VLF OSA 3 Portion, and the geomembrane <br />installed in the Leak Detection Trenches. <br />In accordance with the AMEC Geosynthetics CQA Plan (Section 1400.2 of the <br />Specifications), when an AMEC monitor observed deficiencies, the monitor determined the <br />nature and extent of the problem and notified Lone Star and /or CC &V. When unsatisfactory <br />test results identified a deficiency, AMEC performed additional tests to define the extent of <br />the deficient area. Lone Star corrected deficiencies to meet the requirements of the <br />specifications, and AMEC retested and/or approved the corrected deficiency prior to any <br />additional related work. <br />CQA activities conducted by AMEC during the installation of the LLDPE geomembrane in <br />the 2011 Phase 5 VLF OSA 3 Portion consisted of the following: <br />• Conformance sampling and testing of the geomembrane; <br />• Observation and approval of the SLF surface prior to deployment of 80- <br />mil single -sided textured LLDPE geomembrane; <br />• Observation and documentation of geomembrane deployment, including <br />verifying sheet thickness and overlap, and documenting panel layout; <br />• Observation of trial seam sample testing and documentation of results; <br />• Observation and documentation of field seaming; <br />• Identification of destructive seam sample test locations, observation of <br />destructive seam sample testing, and documentation of test results; <br />• Observation and documentation of geomembrane repairs; and <br />• Observation and documentation of non - destructive pressure testing of <br />fusion seams and vacuum testing of extrusion seams and repairs. <br />The following sections present a detailed description of each of the above activities. <br />AMEC Earth & Environmental, Inc. <br />2000 S Colorado Blvd 2 -1000 <br />Denver, CO 80222 <br />Tel 303 - 935 -6505 <br />Fax 303 - 935 -6575 www amec.com 28 <br />
