ITEM 2: DESCRIPTION OF REPORTING SYSTEM <br />Internal Notification <br />The Environmental Technician for Cotter's Western Slope <br />Operations is responsible for maintaining surveillance over the <br />facilities. Accordingly, he would discover a spill and maintain <br />responsibility for reporting the spill to the Western Slope Mine <br />Manager and the Corporate Supervisor of Environmental Affairs. <br />The Corporate Supervisor of Environmental Affairs will be <br />responsible for agency notifications. <br />Agency Notification <br />If for some unforeseen reason any unusual spills occur, the <br />Supervisor of Environmental Affairs will promptly notify the <br />Colorado Department of Health, Water Quality Control Division and <br />the Environmental Protection Agency by telephone. A written <br />explanation will follow within five days after the occurrence. <br />All records regarding samples collected, analysis, etc., will be <br />maintained at the Cotter Corporation, Western Slope Operations' <br />Nucla Office. These records will be open for inspection upon the <br />request of governmental agencies. <br />3 Considering the distances water spills must travel, any <br />danger to downstream water uses will be determined by the <br />Colorado Department of Health. <br />Materials Containment Plan - Page 4 <br />
