MINESITE INSPECTION NARRATIVE 6 <br /> provisions of the permit, the provisions of the <br /> Colorado Surface Coal Mining Reclamation Act, and the <br /> Regulations of the Colorado Mined Land Reclamation <br /> Board for Coal Mining. <br /> Insurance---The certificate of insurance was issued on <br /> 08/30/92 and expires on 08/30/93 . Mid-Continent <br /> Resources is listed as the insured with liability <br /> coverage in the amount of $2,000, 000.00 general <br /> aggregate. The certificate did not list the <br /> operations insured nor was the cancellation language <br /> considered to be adequate. DMG stated that the copy <br /> on file in their office contains the correct and <br /> required information. <br /> Ownership and Control---Pitkin Iron Co. , is the <br /> current operator at the Coal Basin Mine. This <br /> information was provided by Diane Delaney who stated <br /> that her salary and the salaries of other employees <br /> working at the mine are paid by Pitkin Iron. Ms. <br /> Delaney makes the day-to-day operational decisions on <br /> the site and represents the mine during inspections <br /> and board hearings. Pitkin Iron and its officers and <br /> directors are not identified in the mine plan. This <br /> information is required under Rules 2 . 03 .4 (3) and <br /> 1. 04 (83a) (b) (ii) . This requirement is addressed as <br /> violation number 2 in Ten-Day Notice (TDN) 93-02-352- <br /> 006 TV3 . <br /> Subsidence Monitoring---A permit revision in 1986 <br /> allowed the subsidence monitoring to be discontinued. <br /> Subsidence was measured in 1988 in preparation for an <br /> application for permit renewal. At that time the <br /> monitoring point with the greatest amount of <br /> subsidence was TP-6 which had subsided 2 .7 feet since <br /> 1985. The total subsidence for this point is 6. 29 <br /> feet. <br /> Pond Inspections---Thirty-three ponds, including the <br /> pond at the loadout, are inspected on a quarterly <br /> basis. The ponds that are the subject of DMG Notice <br /> of Violation (NOV) C-93-098, issued this inspection, <br /> do not appear on the inspection reports. These ponds <br /> will also have to be inspected on a quarterly basis. <br /> Reports are up-to-date for the last year. <br />
