MINESITE INSPECTION NARRATIVE 7 <br /> Refuse Pile Inspections---There are three waste piles <br /> on the mine. The Sutey and rock tunnel piles are <br /> being inspected on a quarterly basis. The old refuse <br /> pile is only being inspected twice a year. DMG stated <br /> that the semi-annual basis for the old refuse pile was <br /> approved in the permit. Inspections of the old refuse <br /> pile were not done in the first quarter of 1993, and <br /> third quarter of 1992, out of the last four quarters. <br /> The approved Colorado program requires quarterly <br /> inspections, therefore, violation 1 of the TDN was <br /> issued for this non-compliance. <br /> The inspections indicate that waste was last added to <br /> the Sutey pile on 1/25/91. The reports mention <br /> unstable areas on the west side and southwest edge of <br /> the Sutey pile, and a small area on the east side of <br /> the Dutch Creek flume. <br /> Water Monitoring--- Surface and Ground water <br /> monitoring records were reviewed for 1992 through the <br /> present. Mid-Continent Resources still has the <br /> responsibility to monitor surface and groundwater <br /> according to their approved plan because the <br /> reclamation bond for the mine has not been forfeited. <br /> In addition, the operator is required to have an NPDES <br /> permit covering the discharge points from the mine. <br /> The permit for the mine, CO-0000396, is valid until <br /> December 31, 1993 . Per Ms. Delaney, a request for <br /> renewal of the permit has been submitted to the <br /> Colorado Department of Health. Discharge records for <br /> the first quarter of 1993 were reviewed. There were <br /> no reported violations. Discharge sampling for the <br /> second quarter of 1993 had been done but the lab <br /> results were not posted as of this inspection. <br /> A required annual hydrology report, the latest being <br /> 1992, has been submitted to the Division for review. <br /> A copy of the report was also on file near the <br /> minesite. The operator is required to sample surface <br /> water bi-annually from 5 different sites, Station #20- <br /> Coal Creek, Station 15-Dutch Creek, Station #3-Bear <br /> Creek, Station #3-Sutey Creek and Disney Creek in <br /> addition to the discharge points. They also have <br /> committed to sampling the spring beneath the Sutey <br /> refuse disposal pile. Field parameter testing is <br /> required for measuring flow, temperature, Ph and <br /> conductivity. Grab samples collected from the same <br /> stations require testing for 11 different parameters. <br /> Ground water sampling is done at 10 different sites <br />
