August 28, 2012 C-1994-082/Yoast Mine SLB <br /> Pond 014 was holding water approximately 18"below the discharge level. Pond 013 also held water <br /> about 6"below the riser top. Pond 012 was discharging clear water. Pond 011 held water 10' below the riser top. <br /> Pond 012A held water about 18"below the riser top. Pond 011A held water 10"below the riser top. Pond 010 <br /> was holding water 3' below the riser top. All embankments and spillways were in good condition,with the <br /> exception of Pond 012A. The trash rack on Pond 012A needed to be cleared, and the emergency spillway rip rap <br /> needed to be reworked, as documented in the July 18, 2012 inspection report. <br /> The operator indicated that Ponds 012 and 012A had been surveyed to determine water and sediment storage <br /> capacity and whether the ponds required cleaning,but the data had not yet been analyzed. In the event that the <br /> ponds require cleaning,the work needs to be completed before the end of this construction season to ensure <br /> adequate capacity for spring runoff. <br /> Stock tank ST-1 held water but was not discharging. The surge structure above YPM-8 was dry. <br /> ROADS—Rule 4.0311 Construction 4.03.l(3)/4.03.2(3)11 Drainage 4.03.l(4)/4.03.2(4)11 Surfacing and <br /> Maintenance4.03.1(5) and(6)/4.03.2(5) and (6)11 Reclamation 4.03.1(7)/4.03.2(7): <br /> Road A was in good condition, ditches and culverts were functional. Maintenance items identified during <br /> the August 8, 2012 inspection had not been addressed. A large gully had formed in LU-3A,where YPM6-2 <br /> crosses the road. The road and drainage need to be repaired. LU 3B was in good condition; water bars were <br /> functional. The operator indicated that road maintenance would be done this construction season. <br /> During the August 8, 2012 bond release inspection,the width of LU3A was measured in several locations between <br /> YPM-8 and stock tank ST-1. No width measurements were recorded west of ST-1. During this inspection,two <br /> additional width measurements were recorded. <br /> I I' at the point near shrub plot 2006 31,where the road splits off to a two-track trail down to Pond 013. <br /> 10'8"at the well cluster where LU3A terminates. <br /> REVEGETATION—Rule 4.15❑ Vegetative Cover; Timing: <br /> Revegetated areas were in good condition. Cover was good in the areas walked on the east end of the <br /> north pit and south end of the south pit. No active erosional features were observed in these areas. Sagebrush was <br /> heavy in places, and pockets of Wood's rose were observed. Weed concentrations in the reclaimed areas were <br /> reduced from last year; observations of large weed infestations were primarily made in drainages and along more <br /> recent erosional features or recently disturbed areas. <br /> Shrub seedlings planted in the 2003 plot in the North pit were surviving and had leafed out at the bases. Weeds <br /> were heavy in this plot,particularly houndstongue,thistle, and kochia. <br /> SLIDES and DAMAGE -Rule 4.12: <br /> The Neck Pit, Pond 012 and Pond 013 slide areas all appeared to be stable with no evidence of recent <br /> movement. Rills have formed on the steep Pond 012 cut slope,but the slope itself appears to be stable. The <br /> operator has proposed the use of rock mulch(Minor Revision 39)to stabilize this slope and promote revegetation. <br /> It was not clear whether this work would be completed yet this fall. <br /> Number of Partial Inspection this Fiscal Year: 3 <br /> Number of Complete Inspections this Fiscal Year: 1 <br /> Page 3 of 7 <br />
