"Wilm
A concrete plug was placed at Underground Working #4044. Approximately twenty cubic yards of
approved concrete from Transit Mix was placed in the previously approved plug, shaped in the
shaft. The concrete placement was observed by an AMEC field representative.
Testing of the concrete was performed by an AMEC field representative. Slump, air entrainment,
temperature testing was performed with results passing. One set of nine 4 "x 8" cylinders were
cast for laboratory compressive strength testing.
Remediation of Underground Working #4071 was performed using a John Deere excavator to
place coarse shaft backfill in the open shaft. The working was backfilled to approximately 10 feet
from the existing ground surface. The sides were then shaped with 05V:1 H slopes preparing the
shaft for concrete plug installation. An AMEC field representative observed the backfill and
inspected the shaped upper portion of the shaft. It was found to be acceptable for concrete
placement.
D) Structures
No work performed during the week.
III) Geosynthetics
A) Geomembrane Installation
No work performed during the week.
B) Anchor Trench
No work performed during the week.
C) Geomembrane Exposure
No work performed during the week.
D) Geomembrane Acceptance
No work performed during the week.
IV)Storm Water Management
A) Best Management Practices (BMP)
Best Management Practices are being observed throughout the Mill Site Earthworks project.
CQA ACTIVITIES:
SUMMARY:
The CQA activities performed consisted of observing, documenting and recording the construction
activities of the contractor.
AMEC observed topsoil stockpile shaping.
AMEC observed clear and grub activities performed by Seedmasters within the Mill Site Earthworks
project.
AMEC observed subgrade preparation within the Mill Site Earthworks project prior to overburden fill
placement.
