"Wilm <br />A concrete plug was placed at Underground Working #4044. Approximately twenty cubic yards of <br />approved concrete from Transit Mix was placed in the previously approved plug, shaped in the <br />shaft. The concrete placement was observed by an AMEC field representative. <br />Testing of the concrete was performed by an AMEC field representative. Slump, air entrainment, <br />temperature testing was performed with results passing. One set of nine 4 "x 8" cylinders were <br />cast for laboratory compressive strength testing. <br />Remediation of Underground Working #4071 was performed using a John Deere excavator to <br />place coarse shaft backfill in the open shaft. The working was backfilled to approximately 10 feet <br />from the existing ground surface. The sides were then shaped with 05V:1 H slopes preparing the <br />shaft for concrete plug installation. An AMEC field representative observed the backfill and <br />inspected the shaped upper portion of the shaft. It was found to be acceptable for concrete <br />placement. <br />D) Structures <br />No work performed during the week. <br />III) Geosynthetics <br />A) Geomembrane Installation <br />No work performed during the week. <br />B) Anchor Trench <br />No work performed during the week. <br />C) Geomembrane Exposure <br />No work performed during the week. <br />D) Geomembrane Acceptance <br />No work performed during the week. <br />IV)Storm Water Management <br />A) Best Management Practices (BMP) <br />Best Management Practices are being observed throughout the Mill Site Earthworks project. <br />CQA ACTIVITIES: <br />SUMMARY: <br />The CQA activities performed consisted of observing, documenting and recording the construction <br />activities of the contractor. <br />AMEC observed topsoil stockpile shaping. <br />AMEC observed clear and grub activities performed by Seedmasters within the Mill Site Earthworks <br />project. <br />AMEC observed subgrade preparation within the Mill Site Earthworks project prior to overburden fill <br />placement.
