L WTI I=, k <br />B) Drilling <br />Confirmatory drilling was performed at Underground Working #4008. A drill pattern was set out by <br />an AMEC field representative. Twelve holes were drilled encountering no voids. An approximate <br />total of 600 lineal feet was drilled. As -built survey of the drill holes was performed by Ames. <br />Confirmatory drilling was performed at Underground Working #4047. A drill pattern was set out by <br />an AMEC field representative. Three holes were drilled encountering no voids. An approximate <br />total of 150 lineal feet was drilled. As -built survey of the drill holes was performed by Ames. <br />Confirmatory drilling was performed at Underground Working #4052. A drill pattern was set out by <br />an AMEC field representative. Fifteen holes were drilled encountering fourteen voids. An <br />approximate total of 750 lineal feet was drilled. As -built survey of the drill holes was performed by <br />Ames. <br />Confirmatory drilling was performed at Underground Working #4071. A drill pattern was set out by <br />an AMEC field representative. Twenty -one holes were drilled encountering no voids. An <br />approximate total of 1,050 lineal feet was drilled. As -built survey of the drill holes was performed <br />by Ames. <br />Confirmatory drilling was performed at Underground Working #4073. A drill pattern was set out by <br />an AMEC field representative. Thirty -five holes were drilled encountering no voids. An <br />approximate total of 1,750 lineal feet was drilled. As -built survey of the drill holes was performed <br />by Ames. <br />Grouting of "miss" (non -void) confirmatory drill holes was performed at Underground Workings <br />#4001, #4008, 4052, and #4071. A grout pump and laborers were used to place bentonite grout <br />into the "miss" drill holes. <br />Q Remediation <br />A John Deere 850 excavator was used to excavate the blasted material from Underground <br />Working #4007. The working was excavated to the limits of the excavator. Backfilling was then <br />performed with coarse shaft backfill. Due to access, the material as placed in small lifts and <br />bucket tamped using the excavator. This was performed preparing for geogrid placement and <br />activities were observed by an AMEC field representative. <br />Remediation of Underground Working #4008 (Morning Glory)was performed using a John Deere <br />excavator to place coarse shaft backfill in the open shaft. The working was backfilled to <br />approximately 10 feet from the existing ground surface. The sides were then shaped with 05V:1 H <br />slopes preparing the shaft for concrete plug installation. An AMEC field representative observed <br />the backfill and inspected the shaped upper portion of the shaft. It was found to be acceptable for <br />concrete placement. <br />A concrete plug was placed at Underground Working #4024. Approximately eighteen cubic yards <br />of approved concrete from Transit Mix was placed in the previously approved plug, shaped in the <br />shaft. The concrete placement was observed by an AMEC field representative. <br />A concrete plug was placed at Underground Working #4025.
