VENDOR OFFSET <br /> Pursuant to C.R.S. 24-30-202.4 (as amended), the State Controller may withhold debts owed to <br /> state agencies under the vendor offset intercept system for: (a) unpaid child support debt or child <br /> support arrearages; (b) unpaid balance of tax, accrued interest, or other charges specified in <br /> Article 22, Title 39, C.R.S.; (c) unpaid loans due to the student loan division of the Department of <br /> Higher Education; (d) owed amounts required to be paid to the unemployment compensation <br /> fund; and (e) other unpaid debts owing to the State or any agency thereof, the amount of which is <br /> found to be owing as a result of final agency determination or reduced to judgment as certified by <br /> the controller. <br /> Bid Award <br /> The apparent low bidder must sign and submit the attached Bid Schedule, and a certificate of <br /> insurance with the following limits: <br /> 1. Standard Workers Compensation and Employer Liability, as required by State Statute <br /> including occupational disease, covering all employees at the work site. <br /> 2. General liability (Public Liability and Property Damage) (Minimum): <br /> a. Combined single limit of$600,000 written on an occurrence basis; <br /> b. Any aggregate will not be less than $1 million; <br /> c. The CONTRACTOR must purchase additional insurance if claims reduce the annual <br /> aggregate below $600,000; <br /> d. The State of Colorado must be named as additional insured on each comprehensive <br /> general liability policy; <br /> e. Certificates of insurance must be provided to the state within ten (10) working days after <br /> receipt of award; and, <br /> f. Insurance shall include provisions providing notification to the State ten (10) days prior to <br /> cancellation of the coverage. <br /> 3. Automobile Liability (Minimum). The CONTRACTOR must carry a minimum of$600,000 <br /> combine single limit. <br /> Project Work Description <br /> The Genera!Conditions for Purchase Orders and Purchase Order Terms and Conditions apply to <br /> all work. <br /> The purpose of the project is to perform maintenance at the mine site during and after snow melt <br /> runoff. Maintenance activities which may be conducted include removal of snow, snow slide or <br /> landslide materials from roads, ditches or streams; removal of debris from culverts at stream <br /> crossings; maintenance of roadside ditches and culverts; stream bank protection as necessary to <br /> minimize flood and erosion potential, and maintenance of the Dutch Creek Diversion. <br /> CONTRACTOR may be required to obtain and place rip rap materials. Silt fence or straw bales <br /> may be placed at various locations around the mine site to minimize sediment transportation. <br /> Erosion control work including construction of earthen berms or diversions, and grading of rills <br /> 2 <br />
