r
. 1
V;olation C-91-016
Attachment A
Portions of the Operation to Which This Notice Aoolies:
Road C
1 . RD-5 ditch relief drain at Sta. 122+20. Flow has cut under and around
large riprip in steep embankment.
2. Severe erosion below RD-4 ditch relief drain at Sta. 102+10. Initial
20 feet of channel below outfall has been stabilized , but erosion
continues for a considerable distance down the timbered slope below. The
gully is up to 15 feet wide by 20 feet deep in the 100 foot segment below
the riprapped drain.
3. At Sta. 92+00, gullying 3 to 4 feet deep has occurred for several hundred
feet below the RD-3 outfall , although the riprapped ditch relief drain
itself appears stable.
4. At Sta. 77+20, gullying 4 to 6 feet deep extends for several hundred feet
through an aspen grove below Culvert C-5 outfall , although the riprapped
downdrain segment immediately below the culvert outlet appears stable.
S. 'An un-numbered 24" culvert at Sta. 50+00 discharges into an outlet
channel which makes an abrupt right angle turn several feet below the
outlet. This causes sediment deposition at the outlet and has caused a
recurring maintenance problem.
Road D
6. Gullies 3 to 6 feet deep have developed below culvert outlets at
117+00, Culvert 0-5, and culvert at 105+00. The erosion is on original
ground below the road fill . Gullies originating from the upper two
culverts terminate at the BAT ponds which have filled in with sediment.
The gully from the lower culvert bypasses the BAT ponds and dissipates in
a meadow several hundred yards below the road fill .
7. Minor gully erosion has occurred below the RD-2 outfall at Windy Point.
Below the riprapped down drain, the channel splits into three separate
gullies averaging 1 1 /2 to 2 feet deep. These gullies extend for several
hundred yards down the hillslope .
8. Culvert 0-2N has not been installed .
9. Culvert D-lN has not been installed .
