To assure protection of near -by public water intake structures to the extent practicable <br />and prior to initiation of any construction activities within waters of the U.S., you shall <br />notify Mr. Rick Barth, Rifle City Engineer of your construction activity start and end <br />dates. He can be reached at (970) 665 -6559 or by email at: rbarth(airifleco.org. We note <br />your work schedule dates of mid February to April 30, 2012. <br />3. Temporary coffer dam and diversion channel activity materials can be manipulated by <br />bull- dozer, track hoe or front-end loader equipment until permanent structures (i.e., j- <br />hooks, vanes, toe logs, bank full benches and rip -rap) are installed. Removal of the <br />temporary coffer dam and reshaping the diversion channel and point bar materials <br />(approximately 37,000 ft) will result in this area performing as a floodplain for future <br />high flows. Materials excavated from the river bed associated with the 1,000' x 250' single <br />channel designs submitted (approximately 250,000 ft) will be removed by track hoe <br />and/or front -end loader, transported to an upland location, and prevented from future <br />river recapture. <br />4. To verify permit compliance with submitted designs and structure locations, prior to <br />initiation of construction activities within waters of the U.S., you shall submit to the <br />Corps, pre - construction site photographs of the project site. The photos shall be from set <br />reference locations and used in post - construction comparison. <br />5. Within 30 days following completion of the authorized work you shall submit post - <br />construction site photographs of the project site, as -built drawings, and a description of <br />the work conducted on the project site to this office for review (i.e., post - construction <br />report). The drawings shall include the following: <br />a. The Department of the Army Permit number. <br />b. A plan view drawing of the location of the authorized work footprint (as shown on the <br />permit drawings) with an overlay of the work as constructed in the same scale as the <br />attached permit drawings. <br />c. A description and list of all deviations between the work as authorized by this permit <br />and the work as constructed. Clearly indicate on the as -built drawings the location of <br />any deviations that have been listed. <br />6. You are responsible for all work authorized herein and ensuring that all contractors and <br />workers are made aware and adhere to the terms and conditions of this permit <br />authorization. You shall ensure that a copy of the permit authorization and associated <br />drawings are available for quick reference at the project site until all construction <br />activities are completed. <br />7. Post - construction maintenance activities, including but not limited to, woody debris <br />clearing, structure failure replacement or resetting, re- transplanted vegetation efforts, <br />bed load excavation, point bar reshaping and maintenance activities associated with final <br />channel design dimensions, must be submitted to the Corps for review and approval <br />prior to implementation. <br />
