5 (FORM 1 - All information for DRMS use and confidential filing)
G. Total project area to be disturbed 5.00 (acres)
H. Describe the equipment to be used for the prospecting operations:
Hydraulic excavators will be used to excavate the collapsed adit and partially collapsed
shaft. Trench compactors will be utilized to assure compaction and stability of the
corrugated metal pipe (CMP). Water well type drill rig(s) will be utilized to drill the
relatively shallow dewaleriny wells.
I. Describe and locate any structures to be constructed (i.e. stockpiles, ponds, impoundments):
No new structures will be constructed. Existing stockpiles, ponds, and discharge points
will be utilized.
J. Describe anticipated relationship to surface water and groundwater (proximity to streams, penetration of ground
water aquifers):
The adit of the London Butte will not require dewatering. Dewatering of the lower levels will be
under the existing water right (23U1U3b) and discharge permit (GU- 0U3t3334). Mosquito Creek
Groundwater will be sampled for quality testing Work will cease and DRMS will be notified if
any underground activity results in a surface water discharge not covered in this NOI.
IV. OPERATION AND RECLAMATION MEASURES:
1. The Board suggests that a photographic record of the pre - prospecting and post - prospecting conditions be kept by the
prospector. These photos should be taken from the same location and by the same method to clearly show the
pre - prospecting condition of the land and the reclamation efforts. Upon completion of reclamation and request for bond
or surety release, the Board may consider the photos as evidence of adequate reclamation, and thus, be able to act more
quickly on the request for release.
2. Provide a description of the native vegetation of the area to be disturbed, including tree, shrub, and grass communities
of the area. Color photographs, sufficient to adequately represent the ecology of the site and adequately labeled
(including date, orientation and location), may be used in lieu of a written description. Based on the quality of the
photographs, the Division may require additional detail.
No vegetation will be disturbed. Shaft lies within the existing stockpile /pad area of the
mine. The adit also exists within disturbed ground. Existing roads and catch benches will
be utilized to repair the adit. Clean up of the catch benches will provide additional safety in
the working area.
