If this NOI application is approved, the applicant will be <br />required to supply the following information to the Division in the annual report: <br />a.) The location of each test hole. This information will need to be reported in latitude and <br />longitude in degrees, minutes and seconds or in decimal degrees to an accuracy of at least 5 <br />decimal places. The locations may also be recorded in Universal Transverse Mercator (UTM) <br />coordinates. If UTM coordinates are used, you must identify what North American Datum <br />(NAD) was used to log the location coordinates. <br />b.) Photographs of each test hole location prior to excavation and after the hole has been <br />backfilled and reclaimed. This will ensure prospecting activity is occurring and will document <br />reclamation has occurred at each prospecting site over the life of the operation. <br />c.) A description of the reclamation that occurred at each test hole during the year. <br />d.) An updated map showing the location of all test holes excavated and reclaimed to date. <br />Please commit to complying with Rule 5.6 for the annual report and the requirements discussed above. <br />15.) Item 10 on page 6 of the application requires the applicant to describe how the excavations and other <br />disturbances will be reclaimed. Instead, your response to this item is a discussion of the pre- existing <br />mining and prospecting features on the site. Please specifically state how you intend to backfill the test <br />holes and redistribute the topsoil over the hole. <br />16.) Also for Item 10 and 12, please indicate how the processing area will be reclaimed after the <br />prospecting operation is complete. If vegetation and soil will be disturbed as a result of processing, the <br />applicant will be required to revegetate the area. If so, please indicate how the processing area will be <br />prepared for planting and provide the following information: <br />a.) Indicate if the soil will be scarified. <br />b.) Indicate if soil amendments (fertilizer) will be used. <br />c.) If mulch will be applied to the processing area, indicate the type and the amount to be used. <br />d.) Provide a seed mixture and seeding rates to revegetate the processing area if vegetation will be <br />disturbed at the processing area. The local Natural Resource Conservation Service (NRCS) office <br />should be able to provide you with an appropriate seed mixture for the area. <br />Financial Warranty <br />17.) The Division will estimate a cost to reclaim the site based on the application and your response to the <br />review issues above. The financial warranty may or may not exceed $2,000 per acre of affected land <br />depending on the Division's cost estimate. Please be reminded that prospecting operations may not <br />commence until the issues are addressed and the bond is submitted and approved by the Division. <br />
