• Submit revised design plans for portal stabilization, including repair drawings
and specifications, as necessary, through the Technical Revision or
Amendment process;
• After obtaining DRMS approval through the Technical Revision or
Amendment process, proceed with remainder of work plan.

Page 4 fourth paragraph, the applicant references Figure 6.5 -2 for the Incas Road and
Chief Portal geotechnical drill program. The Incas Road is labeled on Figure 6.5 -2, but
not the Chief Portal. The Chief Portal should be labeled on the figure for future
reference and location.

Page 4, New Access Road. The second bullet references "New Access Road Stability
Report (Attachment 6.5 -3 and Attachment 6.5 -4) ". Attachment 6.5 -3 is the
"Geotechnical reconnaissance report" referenced in the first bullet and Attachment 6.5 -4
is the "Analysis of the Effect of Blasting Operations on Structures Associated with
Mining Activity at the May Day Mine. The "New Access Road Stability Report" is not
to be found.

The "New Access Road Stability Report" was inadvertently left out of the
revised August 9, 2011 mine permit application; however the report was included in
previous submittals. Please find the requested report in Attachment 5.

Page 6, Embankments. The reader is directed to Attachment D -5 for the Augmentation
Pond embankment discussion. Attachment D -5 is the Dry Stack Tailings Work Plan.
The Augmentation Pond work plan is presented in Attachment D -6.
