3.6 OVERALL QA <br />A. QA personnel will perform observation and verification testing of <br />construction materials, workmanship, and Contractor QC activities. QA <br />activities shall include cross checking of QC testing frequency, methods, <br />procedures, and personnel in accordance with the Design Drawings and <br />Specifications. <br />3.7 NONCONFORMANCE AND FAILED QA TESTING <br />A. Rio Grande Silver shall notify the Contractor of any detected <br />nonconformance with the foregoing requirements. After receipt of such <br />notice, the Contractor shall immediately take corrective action. When <br />delivered to the Contractor at the site, such notice shall be deemed <br />sufficient for the purpose of notification. <br />B. If a QA test fails during the construction phase, the Contractor may <br />submit a recheck sample for additional testing. If a recheck sample fails, <br />the material shall be rejected. Recheck sample testing shall be at the <br />expense of the Contractor. Nonconformance and failed QA testing <br />procedures shall be addressed in conformance with the CQCP. <br />C. When deficiencies (items that do not meet project requirements) are <br />discovered, the CQA Monitor shall immediately determine the nature and <br />extent of the problem and notify the Installer or Contractor. If <br />unsatisfactory test results identify a deficiency, additional tests will be <br />performed to define the extent of the deficient area. <br />D. The Installer or Contractor shall correct the deficiency to the satisfaction <br />of the CQA Monitor. If the Contractor is unable to correct the problem, <br />the CQA Monitor will notify the Project Manager and Design Engineer <br />who will assist in problem resolution. If the solution involves a design <br />revision, the Design Engineer shall also be contacted. <br />E. The corrected deficiency shall be retested and /or approved by the Project <br />Manager and Design Engineer before any additional related work is <br />performed by the Installer or Contractor. All retests and related <br />documentation shall be recorded by the CQA Monitor and included in the <br />final Construction Certification Report. <br />3.8 GEOSYNTHETIC QA <br />A. Construction of the HDPE Liner or specified geosynthetics must be in <br />compliance with the design Drawings and Specifications. Rio Grande <br />Silver shall implement a CQA monitoring and testing program to ensure <br />construction compliance by the Contractor. The QA program shall consist <br />of reviewing Contractor QC submittals, material conformance testing, and <br />construction monitoring and testing. <br />Rio Grande Silver Tailings Evaporation Pond 01450 -9 Quality Assurance /Quality Control <br />August2011 01450 <br />
