New Access Road Construction Work Plan <br />Exhibit D — Attachment 1 <br />Implement Post Construction Monitoring Program <br />• Monument control points will be established at 20 +20, 20 +40, 20 +60 and 20 +80. <br />(Attachment A at Sheet C3.10). Following baseline readings, first round of survey <br />measurements will be obtained one week later and quarterly thereafter while road is <br />in use. <br />• A monitoring program to measure the magnitude and rate of movement of key access <br />road locations and retaining wall components will be designed and implemented by <br />the Geotechnical Engineer. <br />Inspection and Maintenance Program <br />On monthly basis: <br />• Remove debris, slash, and trees to a temporary storage area located at 22 +40; <br />• All paper, tape, wood lathes etc will be bagged and disposed in an offsite landfill; <br />• Inspect retaining wall, roadway for signs of distress; <br />• Check for debris in roadway, remove as necessary; <br />• Check condition of silt fence at base of slope; remove soil /rock buildup and repair <br />any damage observed as necessary; and <br />• Visually check and record cracks in roadway. <br />Reporting: <br />• Prior to work activity obtain photographs of the project site; <br />• Document daily work activities such as: <br />o Environmental engineering construction monitoring records; <br />o Clear area and dispose debris, trash, and paper offsite; <br />Wildcat Mining Corporation <br />May Day Idaho Mine Complex — 112d Permit Application <br />Revised August, 2011 <br />Exhibit D — Attachment 1 <br />o The monitoring points will be monitored monthly by a surveyor licensed to <br />practice in the State of Colorado to within a tolerance of 0.01 feet. Where <br />movement exceeds 0.1 feet between monthly readings, monitoring frequency <br />will be reduced to weekly. If movement exceeds 0.1 feet between weekly <br />readings, use of the access road shall cease until an inspection by the <br />Geotechnical Engineer is performed to assess the nature of the movement <br />judges the road sufficiently stable or until recommended repairs implemented <br />under his/her oversight; and <br />o Inspection and mitigation reports generated by the Geotechnical Engineer will <br />be issued to the Regulatory Authority within 1 week of the activities being <br />documented. <br />Page 13 of 15 <br />
