New Access Road Construction Work Plan <br />Exhibit D — Attachment 1 <br />• Observing retaining wall foundation excavations and roadway over excavation <br />to check that suitable bearing materials are exposed and proper cleanup <br />achieved; <br />• Performing the necessary laboratory testing to check that construction <br />materials, including fill and backfill materials, conform to the specifications; <br />• Observing the placement of fill material and performing in -place field density <br />tests to check that the specified compaction requirements are achieved. Field <br />density tests will be performed in accordance with ASTM 2922 -05. Tests will <br />be performed in each 12 inch layer placed at a frequency not to exceed 1,000 <br />square feet/test; <br />• Observing the construction of retaining walls and performing the necessary <br />field and laboratory tests to check the walls are constructed in accordance with <br />the construction drawings and specifications; and <br />• Reviewing the Contractor's daily reports and other quality control information <br />to check that work is being performed to the prescribed standards and that <br />unanticipated issues arising during construction are promptly addressed. <br />Project Closeout <br />Documentation <br />Wildcat Mining Corporation <br />May Day Idaho Mine Complex — 112d Permit Application <br />Revised August, 2011 <br />Exhibit D — Attachment 1 <br />• Contractor will notify the Geotechnical Engineer of substantial completion with five <br />days of completion. The Geotechnical Engineer will then schedule a site walkover <br />with the Contractor and Regulatory Authority. Work remaining and/or unsatisfactory <br />work product identified by Geotechnical Engineer and Regulatory Authority will be <br />discussed with the Contractor and memorialized in a memorandum identifying the <br />`punch list' items that it needs to satisfactorily addressed <br />• Minutes of the Kick -Off Meeting will be issued to attendees within three days of its <br />being held. Daily reports generated by the Contractor will be reviewed by the <br />Technician or engineer of record on a daily basis. The Technician will prepare daily <br />reports describing his/her observations and actions taken to correct deficiencies in the <br />work. Field test results and the Contractor's daily reports will be included as <br />attachments. The Technician's daily reports will be forwarded to the Geotechnical <br />Engineer for review and action (if any is required). The Geotechnical Engineer will <br />transmit the daily reports to the Regulatory Authority on a weekly basis along with a <br />brief summary of the work, including actions taken, outstanding issues to be resolved <br />if any, and planned activities for the upcoming week. <br />• At the completion of the work, a construction completion report will be prepared that <br />provides a brief description of the work performed and a certification that the work <br />was performed in accordance with the construction drawings and specifications. <br />Attachments to the construction completion report will include as -built drawings, and <br />the results of field and laboratory tests performed during construction. <br />Page 12 of 15 <br />
