Mining Plan <br />Exhibit D <br />Wildcat Mining Corporation <br />May Day Idaho Mine Complex — 112d Permit Application <br />Revised August, 2011 <br />Exhibit D — Rule 6.4.4 <br />1. Phase 1 - Preliminary Construction and Reclamation Activities <br />A. Access Road and Staging Area Construction: <br />Wildcat will modify and stabilize the New Access Road from County Road 124 <br />(minimum 12 foot wide) to the bridge in accordance with the work plan and CLC <br />Associates access road design presented in Attachment D -1 and Exhibit 6.5. <br />Several staging areas are planned for the operation. The Base of Operations, including <br />the helicopter pad as suggested by the Fort Lewis -Mesa Fire District, will be constructed <br />on the west side of the La Plata River located approximately 150 feet east of CR -124. <br />The Base of Operations will be the location where supplies and the support from outside <br />service contractors (sewage, potable water delivery, fuel, oil products, chemicals, mine <br />equipment, emergency and safety equipment) will be transferred to mine personnel and <br />mine equipment for storage or distribution. The staging area at the Base of Operations <br />(See Attachment D -1, Figure C6.10, and Exhibit C -4) will consist of locked chemical and <br />fuel storage units with the secondary containment needed to manage issues related to <br />potential spills and erosion and sedimentation impacts (See Exhibit U for containment <br />details). The Base of Operations will include 5 parking spaces with additional room to <br />maneuver field and delivery trucks. A helicopter pad will be constructed to support <br />emergency response activities. The staging area at the Base of Operations will be fenced, <br />gated, and/or bermed to limit access and to provide a visual and noise barrier. <br />A second staging area will be located at the base of the New Access Road to provide <br />room to support New Access Road construction activities. This staging area will be used <br />to temporarily stage a mobile crusher and store road construction material (rock, culverts, <br />etc.). The staging area will not be located in the designated wetlands (See Attachment D- <br />1 and Exhibit 6.5 for more details). <br />A third staging area will be located at the May Day No. 2 bench. This staging area will <br />be used to temporarily store underground mine equipment and supplies. The bench will <br />also be used to store potable water, spill response kits, extinguishers and house a Port-a- <br />let. In addition, five parking spaces will be made available. <br />B. Maintenance of Existing Access Roads, Expansion of May Day No. 2 Bench and <br />Installation of Stormwater Best Management Practice Structures <br />Where necessary, Wildcat will perform maintenance on existing access roads prior to <br />other construction activities. As necessary, the May Day No. 1 west bench and May Day <br />No. 2 bench will be used to temporarily store surface grading material and run of mine <br />rock that is non -toxic or not acid - producing. <br />The Storm Water Management Plan (SWMP) is included in the EPP.
