Vehicle maintenance wastes, including used oils and other fluids, shall be handled and <br />managed by personnel trained in the procedures outlined in this plan. <br />14. Fuels shall be dispensed by authorized personnel during day light hours only. Fuel <br />dispensing operations shall be attended by authorized personnel only. Personnel must <br />be stationed at both ends of the hose during fueling unless both ends are visable and <br />readily accessible by one person. <br />15. Fuel dispensing equipment shall be of the appropriate and approved type. <br />16. Immediately upon learning of any fuel, oil, hazardous material or other regulated <br />substance spill, or upon learning of conditions that will lead to an imminent spill, the <br />person discovering the situation shall initiate action to eliminate the source of the spill <br />to the maximum extent that is safely possible. <br />. The individual must notify the immediate supervisor or the Spill Coordinator and <br />provide them with the following: <br />Location and cause of the spill <br />Type of material that has spilled <br />Whether the spill has reached or is likely to reach any surface water <br />. Upon learning of the spill or potential spill the Spill Coordinator will: <br />Assess the situation and determine the need for further action <br />Direct subsequent activities and or further assign responsibilities to others <br />Procedures regarding the excavation and disposal of contaminated soil <br />The Spill Coordinator shall determine if the spill is beyond the scope of on -site <br />equipment and personnel and if it is shall immediately notify the mining <br />superintendent that an emergency response contractor is required to contain or clean <br />up the spill. <br />17. The Spill Coordinator shall complete a Spill Report Form for each release of a <br />regulated substance, regardless of volume. The Spill Report Form must be completed <br />immediately and submitted to the General Manager of Jubilee Venture within 24 <br />hours. <br />The following information shall be compiled to complete the Spill Report Form: <br />Description of the spill location <br />Type and estimated volume and manufacture's name <br />The media in which the spill exists <br />Topography and surface conditions <br />Proximity of surface waters <br />Weather conditions <br />Name address and phone # of mining superintendent <br />Cause of the spill <br />Immediate containment and cleanup actions <br />
