June 6,2011 C-1 982-057/Seneca II-W Mine SLB <br /> A deep narrow gully had formed through the undisturbed area at the east end of Topsoil Stockpile QQ, <br /> perpendicular to Road G. This will need to be repaired. <br /> ROADS—Rule 4.03❑❑Construction 4.03.1(3)/4.03.2(3)❑❑Drainage 4.03.l(4)/4.03.2(4)❑❑Surfacing and <br /> Maintenance4.03.1(5)and(6)/4.03.2(5)and(6)1-1 El Reclamation 4.03.1(7)/4.03.2(7): <br /> Equipment was working on the narrowing of the Mine Entrance Haul Road(MEHR)and cleanup of the <br /> material in 006 Gulch that resulted from the slide along the road. The operator anticipates that the earthwork <br /> portion of the road narrowing reclamation project will be complete in approximately ten days. <br /> The outfall downdrain of Culvert A5 needs to be maintained where downcutting has occurred. An 8"-10"gully <br /> has developed beyond the rip rap channel at the Culvert PM10 outfall,where PM4C and PM4 converge. The <br /> outlet side of TAHRC 7 was downcutting and in need of repair on the outlet side;the inlet side was also eroding <br /> where runoff from the adjacent agricultural land enters the culvert. TAHRC 12 needed to be cleaned out on the <br /> outlet side; it was about 1/4 blocked. The inlet side of TAHRC 16 was downcutting. TAHRC 18 (at the Savage <br /> Shop entrance)was about half full of sediment and needed to be cleaned out. <br /> Portions of LU5 were affected by cracks and slumps running through the reclamation above channel PM2C. <br /> The road ditch between culvert G3 and Topsoil Stockpile QQ was in need of maintenance,where downcutting was <br /> occurring. The road ditch downdrain between Topsoil Stockpiles PP and QQ had diverted off of the TRM and <br /> will need to be repaired. The ditch on the west side of Road G,between Culvert G5 and Pond 005 was <br /> downcuttting significantly(as much as 3' at the pond inlet)in the unarmored portion of the channel. The upper <br /> 150' of the channel is lined with rip rap,but the downcutting occurs downstream of the rip rap. This area was <br /> discussed with the operator,and it was suggested that the operator might add small check dams along the <br /> remainder of the drainage to reduce the erosive velocities while minimizing any additional disturbance along the <br /> currently undisturbed areas along the channel. <br /> A 1'-2' gully was coming off of the Ridgeline Access Road at the fence line crossing approximate 1,200' north of <br /> the intersection with LU1; the runoff through the gully drained to Pond 015. The same issue exists at an <br /> undisturbed drainage into Pond 009; there is a 1'-2' deep gully coming off the Ridgeline Access Road at LU1; and <br /> 1300' south of the LUl intersection where a 10"gully extends about 200 yards through the undisturbed area. <br /> These areas were included in the discussion with the operator about the possibility of adding dissipating structures <br /> to prevent erosion in undisturbed areas between disturbed areas and sediment ponds. <br /> The operator needs to install silt fence or a catchment basin at the ditch outlet just south of Culvert J-1,where <br /> drainage exits the roadway in to the undisturbed area to the west. The inlet side of Culvert J-4 needed to be <br /> maintained where downcutting has occurred. <br /> REVEGETATION—Rule 4.15❑❑Vegetative Cover;Timing: <br /> White top,houndstongue,and thistle were observed at various areas throughout the site. White top was <br /> observed on Topsoil Stockpile I and Topsoil Stockpile QQ.Thistle was observed on the Pond 016 embankment, <br /> on the 016A slide area shrub plot,the area between Ponds 016 and 016A,near Stock Tank T25,on the inlet side of <br /> Number of Partial Inspection this Fiscal Year: 10 <br /> Number of Complete Inspections this Fiscal Year: 4 <br /> Page 3 of 9 <br />
