Since this surface blasting plan is applicable to surface blasting <br />activities incident to underground mining activities, the written <br />notification will only be given to all residents or owners of dwellings or <br />other structures located within one -half mile of the blast site. <br />Upon request to the Division by a resident or owner of a dwelling or <br />structure that is located within one -half mile of a blast site, the Operator <br />will promptly conduct a pre - blasting survey of the dwelling or structure <br />and promptly submit a report of the survey to the Division and to the <br />person requesting the survey. If a structure is renovated or added to, <br />subsequent to a pre - blasting survey, then upon request to the Division <br />a survey of such additions and renovations shall be performed. <br />The survey shall determine the condition of the dwelling or structure <br />and document any pre - blasting damage and other physical factors that <br />could reasonably be affected by the blasting. Assessments of structures <br />such as pipes, cables, transmission lines, and wells and other water <br />systems shall be limited to surface conditions and readily available <br />data. Special attention shall be given to the pre - blasting condition of <br />well and other water systems used for human, animal, or agricultural <br />purposes and to the quantity and quality of the water. <br />A written report of the survey shall be prepared and signed by the <br />person who conducted the survey. The report may include <br />recommendations of any special conditions or proposed adjustments to <br />the blasting procedure which should be incorporated into the blasting <br />plan to prevent damage. Copies of the report shall be provided to the <br />person requesting the survey and the Division. If the person requesting <br />the survey disagrees with the results of the survey, he or she may <br />notify, in writing, both the permittee and the Division of specific areas of <br />disagreement. <br />( a)(vi) Unavoidable hazardous conditions which could delay a scheduled blast <br />include emergency situations where rain, lightning, or other <br />atmospheric conditions or Operator or public safety concerns warrant <br />unscheduled blasts. <br />(a)(vii) <br />If blasting is conducted in accordance with the following equation, the <br />peak particle velocity shall be deemed to be within the limit.
