May 11, 2011 C-1981-044/Williams Fork Mines JRS
HYDROLOGIC BALANCE (cont.)-The mine No. 5 dewatering ponds, 5-P5, 5-P6 and 5-P7 were very low at
the time of the inspection. The dewatering pump in the mine has been down for some time now. Joe Shoemaker
indicated that the mine received a new pump and that a drill rig had been on site, but it was not clear whether or
not the pump had been replaced. There was a large pile of pipe and a roll of electric cable by the well site. The
pored embankments were well vegetated and stable. No erosional features were noted on the embankments.
- Sediment Pond 5A-P1 contained a small amount of water and was not discharging at the time of the inspection.
The;pond embankment was well vegetated and stable and no erosional problems were noted.
-Culvert 26(on the main haul road by the spoil spring)was clear.
-Culvert 2 (on the west side of the old entrance road)was clear.
-Culvert 3 (on the east side of the old entrance road)was clear.
-Culvert 5 (on the south side of the haul road under Hwy 13)was clear at the inlet side but was approximately half
full of sediment at the outlet side. This culvert needs to be cleaned.
- Culvert 4(on the north side of the haul road under Hwy 13) was clear.
-Culvert 37(the first culvert entering the substation just east of Hwy 13)was clear at the inlet side but was
approximately half full of sediment at the outlet side. This culvert needs to be cleaned.
-Culvert 32 (the second culvert entering the substation just east of Hwy 13)was clear.
-Culvert 19(the culvert at the"Y"to the refuse piles)was clear on the inlet side and was crushed at the outlet
side. This culvert needs to be repaired.
-Culvert 7 (the culvert above the"Y"to the refuse piles)contained some sediment at the inlet side and was clear
at the outlet side. This culvert needs to be cleaned.
-Culvert 50(to the solid waste disposal area)was clear.
-Culvert 52 (at the 7-north dewatering well)was crushed at both the inlet and outlet sides. This culvert needs to
be repaired.
- Culvert 44(south of the bridge on the haul road)was clear.
-Culvert 15 (west of the reclaimed refuse pile)was clear.
-Culvert 24(at the fork between the refuse piles)was clear.
-Culvert 23 (south of the-new refuse pile area)was clear.
- Culvert 42 (runs along the haul road bridge over the Williams Fork River)was clear.
-Culvert 36(at the entrance north of the old B&S building)was clear.
-All.of the roadside ditches, which were observed as the culverts were checked, contained a small amount of
sediment. The ditches were all well vegetated and stable and no erosional problems were noted at the time of the
inspection. The Division recommends that Moffat County Mining inspect all of the ditches this spring and clean
as necessary.
ROADS—Rule 4.03 Construction 4.03.1(3)/4.03.2(3)Drainage 4.03.1(4)/4.03.2(4) Surfacing and
Maintenance4.03.1(5)and(6)/4.03.2(5)and(6)Reclamation 4.03.1(7)/4.03.2(7):
-There is a small gully beginning to form on the haul road at the bridge just south of the 7-North dewatering well.
The gully is approximately 1/3 of the way across the bridge on the west side of the road. This gully needs to be
repaired before it creates a problem for the road. The remaining haul road was stable and well maintained.
Number of Partial Inspection this Fiscal Year: 5
Number of Complete Inspections this Fiscal Year: 4
Page 3 of 6
