Memo to Wally Erickson 2 March 7, 2011 <br />Review of Access Road Geotechnical Stability File No. M-1981-185 <br />• The road stabilization work plan must be clear and specific while addressing the required work to <br />be done, the order in which it is to be completed, and how QA/QC compliance with designs and <br />specifications is to be certified by a qualified licensed professional engineer. <br />The Division has reviewed the geotechnical and stability analysis, and other related information, included <br />with the May Day Idaho Mine Complex, File No. M-1981-185, permit conversion (CN-01) application <br />submittal dated September 29, 2010 and November 10, 2010. <br />The following adequacy issues must be addressed to the satisfaction of the Division prior to the Division <br />recommending approval of CN-01. Furthermore, after CN-01 is issued, the Division must receive, review <br />and accept the As-Built Certifications for the reconstructed road prior to the road being fully approved. <br />Wildcat cannot access the site via the road until the Division accepts the As-Built Certifications. <br />Mining Plan (submitted November 10, 2010) <br />1. The Applicant states, the access road from County Road 124 to the bridge will be constructed and <br />stabilized in accordance with the work plan presented in Exhibit 6.5 - Geotechnical Stability. The <br />Division did not receive a work plan for the access road construction and stabilization. Please <br />provide the work plan to the Division for review. <br />2. The Applicant states, all existing access roads will be regraded prior to other construction <br />activities. Please provide a detailed work plan for the proposed regrading activities. <br />3. The Applicant states, a Stormwater Management Plan (SWMP) has been submitted to the <br />Colorado Department of Public Health and Environment (CDPHE). Please provide the Division <br />copies of the approved Stormwater permit, and the final SWMP, for the access road and Best <br />Management Practices (BMP) structures. <br />Reclamation Plan (submitted November 10, 2010) <br />The Division's review of the road is specific to the statement in the Reclamation Plan by the Applicant; the <br />access road is temporary and will be reclaimed at the conclusion of mining activities. <br />4. The Applicant states on page 4 of the Reclamation Plan, the access road reclamation area will have <br />a side slope not exceeding 1.5(H):1(V) with the exception of the cut slope which will remain at a <br />slope of approximately 03:1. The Applicant states in the cutslope section of Attachment 6.5-2 - <br />Access Road Repairs and Maintenance, "...in the cut slope between Sta. 0+00 to 3+00. Slopes in <br />this interval will be flatted to 1.5:1 using an excavator." Please explain this discrepancy in the <br />proposed slope for the cutslope section of the access road. <br />The Division will require these slopes be laid back a minimum to 1.25(H):1(V) as recommended in <br />the Trautner Geotech report to La Plata County. Please commit to resloping the cut slopes per the <br />Trautner specifications or 1.5(H):1(V) as stated in the Reclamation Plan.
