the five-year warranty period (see requirement under 'Operation and Maintenance' <br />below). <br />c. Certification that roadway settlement due to all six (6) inch overeater boring installations <br />does not exceed 0.4 inches in twenty-five (25) feet, shall be provided not less than thirty <br />(30) days after project completion and again prior to the initial bond expiration date or <br />the bond will automatically be extended for a new two-year period. Once certification is <br />received thirty (30) days prior to bond expiration after the initial or subsequent bonding <br />period(s), the bond will be released. Such certification shall be provided on CDOT Form, <br />and signed by a licensed professional engineer registered in the State of Colorado. <br />Any and all drainage and environmentally-sensitive areas shall be directionally bored. <br />Buried, parallel utility installations shall be placed within four (4) feet of the Right-of-Way, <br />where practicable. <br />Buried, parallel utility installations shall be placed to a minimum cover depth of 48 inches, <br />or 60 inches if within 15 feet of the roadway. <br />Any parallel installation buried within 15 feet of roadway and less than 60 inches in depth shall <br />be capped with a minimum of six (6) inches of concrete. <br />Warning wire or tape shall be incorporated into all backfill operations. <br />All above ground structures (pedestals, marker posts, etc.) shall be placed within one (1) foot of <br />the Right-of-Way line. <br />Valve boxes, manhole covers, hand holes, monitoring well covers, etc. installed off of the <br />roadway shall be set approximately 1/2 inch below finished grade. <br />Any change in property use, use of an existing access, or installation of a new access will require <br />a CDOT Access Permit. Contact Valerie Sword at (719) 546-5407 for further information. <br />NOTE: If the proposed work disturbs one acre or more total surface area, a Colorado Discharge <br />Permit System (CDPS) Stormwater Construction Permit is required from the Colorado <br />Department of Public Health and the Environment. The Pennittee shall be responsible for <br />obtaining this and all required permits. A copy of a the CDPS , shall be provided to the <br />CDOT Region 2's Permit Office, P.O. Box 536, Pueblo, CO, 81002, at least ten days prior to <br />commencement of the work to validate this Utility/Special Use Permit. Failure to acquire <br />and provide a copy of the CDPS to the Department shall void this Utility/Special Use <br />Permit and all facilities installed by the Permittee shall be removed from the Right-of-Way. <br />Contractor shall designate a SWMP Ad min istrato r/Erosion Control Supervisor. <br />Designate/identify the individual(s) responsible for implementing, maintaining and revising <br />SWMP, including the title and contact information. <br />Erosion, sedimentation and water pollution caused by highway construction activities are a <br />concern to the Colorado Department of Transportation (CDOT). <br />Contractor must comply with all applicable permits including Colorado Discharge Permit <br />System (CDPS) permit.
