If frost exists in the permit site, the Permittee shall be responsible for re-compaction, if <br />necessary, when frost is no longer present. <br />All underground utility crossings of highways shall be pushed or bored under the roadway <br />and shoulders. <br />Depth of all underground utility crossings of highways shall be a minimum of four (4) feet <br />below lowest ditch elevation. All encasements shall extend from Right-of-Way line to <br />Right-of-Way line. <br />All underground utility crossings of paved side roads and driveways shall be pushed or <br />bored. <br />Where water, high-pressure gas, or hazardous material pipelines are either in/under the <br />roadway or suspended from a highway structure, a casing pipe shall be required. This <br />casing shall be of sufficient size and type to protect the carrier pipe from damage during <br />installation and provide means of conveying leaking fluids or gasses to points safely beyond <br />the traveled roadway. <br />Unless otherwise specified, buried crossings shall be bored or jacked beneath the roadway, at <br />least from toe of slope to toe of opposite slope. Portals for untrenched crossings more than 5 feet <br />in depth shall be bulk headed in conformance with OSHA construction and safety standards. <br />Portal limits of untrenched crossings shall be established safely beyond the highway surface and <br />clear zone and in no case shall the lateral distance from the surfaced area of the highway to the <br />boring or jacking pit be less than the vertical difference in elevation between such surface and <br />the bottom of the pit. Water jetting or tunneling is not permitted. Water assisted boring may be <br />permitted, as determined by the CDOT Inspector for borings less than six (b) inches in diameter. <br />Boring hole shall be oversized to the minimum amount required to allow pull-through of the <br />conduit being installed. Resultant voids shall be grouted or otherwise backfilled, subject to <br />CDOT approval. Ends of bored sections shall not be covered before being inspected. <br />Encasement shall be consistent with Utility Accommodation Code provisions. CDOT may <br />require protective casing for shallow installations or certain conduit materials. Encased <br />crossings shall extend at least from toe of slope to toe of slope, or the full width between access- <br />control lines on freeways, including the Interstate System. All borings equal to or greater than <br />six (b) inches in diameter shall be designed and signed by a registered professional engineer <br />licensed in the State of Colorado. <br />CDOT will determine the extent of inspection services necessary for a given installation. <br />Permittee shall attend final inspection as may be required. If the initial performance of permitted <br />work was unacceptable, as determined by the Department, the Permittee shall perform any <br />reconstruction or improvement of that work as ordered by the Department, in a timely manner <br />and prior to any further construction. If permitted operations are not being carried out in <br />compliance with the terms and conditions of this permit, the Department may order the Permittee <br />to perform whatever corrective measures are necessary to attain compliance with the permit. If <br />there is an immediate danger to the public's health, safety or welfare, the Department may order <br />the Permittee to cease all operations and if necessary, to remove all equipment and facilities from <br />the SHROW. <br />b. Final acceptance does not relieve Permittee of maintenance obligations toward those <br />elements of the highway facility constructed under this permit.
