CONSTRUCTION <br />The Permittee is hereby advised that other utilities exist within the proposed permit area. <br />Permittee shall implement any and all measures to protect any existing utilities from damage. <br />Utility Notification Center of Colorado (UNCC) shall be contacted at least three (3) days prior to <br />any excavation, for locates of existing utility lines. <br />For locates of CDOT-owned underground facilities, contact Dave Pierce at (719) 546-5499. <br />All work and materials shall meet or exceed the current CDOT Standard Specifications for Road <br />and Bridge Construction. <br />All excavations and tunnels shall meet all requirements of the Occupational Safety & Health <br />Administration (OSHA). <br />All backfill is subject to 100%AASHTO T-99 Standard Compaction. <br />No open cuts of the roadway surface are allowed <br />As required by the State Highway Utility Accommodation Code, 11-17-2008, flowable backfill <br />shall be used as backfill material for all open cuts within the roadway template, from toe of slope <br />to toe of slope. For those excavations deeper than 4 feet, the top 4 feet of backfill shall consist of <br />flowable backfill, after allowing for depth of patch. For those excavations less than 4 feet deep, <br />the entire depth shall be backfilled with flowable backfill, after allowing for depth of patch. <br />All wash out from concrete mixer or pump trucks and drilling fluids from bore operations <br />will be removed from the site, no wash out is permitted on State ROW or left as a result of <br />work performed on State ROW. <br />When open cuts of the roadway surface are allowed, saw cutting is required to provide a straight <br />edge for patching. <br />Final patch surface shall be smooth and conform to the surrounding pavement surface with no <br />bump, dip, or other noticeable difference in the riding quality. On trenched installations unless <br />otherwise specified by the Department, additional cutback of base and surfacing (2") to <br />approximately 4 feet beyond normal trench limits ("T-cap" method) shall be required to <br />minimize late development sag in the patched area. <br />Flowable backfill shall be used as backfill material for all open cuts within the roadway template, <br />from toe of slope to toe of slope. For those excavations deeper than 4 feet, the top 4 feet of <br />backfill shall consist of flowable backfill, after allowing for depth of patch. For those <br />excavations less than 4 feet deep, the entire depth shall be backfilled with flowable backfill, after <br />allowing for depth of patch. <br />Flowable backfill shall meet the requirements of Section 206 of the current CDOT Standard <br />Specifications for Road and Bridge Construction. <br />Maximum layer thickness of flowable backfill is three (3) feet. <br />Revised 10/13/2010 4
