Contractor is also liable lox damage and repair of construction related erosion, sedimentation and <br />or water pollution as a result of the activities in or adjoining the SHROW. Liability may be up to <br />and including two years from completion of activities. <br />Areas of Right-of-Way disturbed during this installation shall be restored to original contour and <br />condition. This requirement includes: grading to restore drainage, placement of topsoil, and <br />fertilization, seeding, and mulching with approved material, at rates specified in the CDOT <br />Standard Specifications for Road and Bridge Construction. <br />All seed and mulch shall be certified as being free of noxious weeds. Certification shall be <br />provided upon request by CDOT. <br />All seeding and mulching equipment shall be cleaned prior to delivery to the permit site. <br />MISCELLANEOUS <br />A valid Certificate of Insurance, showing CDOT as the certificate holder, shall be <br />submitted prior to beginning work. <br />Any damage to CDOT facilities shall be repaired prior to continuing with other permit work. <br />Minor modifications made necessary by field conditions may be approved by the Maintenance <br />Inspector. All major modifications require the written approval of the Area Maintenance <br />Foreman or Utility Engineer. <br />The expiration date of this permit is noted on the permit. Any request to extend the time frame <br />of this permit shall be made in writing. Submittal ofan additional permit maybe required. <br />In the event it becomes necessary for CDOT to utilize the _portion of Right-of-Way represented <br />by this permit, it is a condition of this permit that the facility be removed and /_or relocated <br />within 30 days of notification by CDOT. leaving the hermit site in it's original condition. <br />All procedures and work are subject to inspection by CDOT personnel. <br />No permit work shall be permitted at night, weekends, or holidays, without prior authorization <br />from the Area. Maintenance Foreman or Utility Engineer. <br />Any CDOT monuments/markers are the sole responsibility of the Permittee to find, mark, catalog, & <br />replace if necessary anywhere within the scope of the permitted project. Construction may need to be re- <br />routed to avoid disturbing High Accuracy Reference Network Survey Land Markers, Benchmarks, or <br />other survey marks. For further information, you may contact Tom Adams, Region 2 Survey <br />Coordinator, (719) 546-5454, email "tnni.adams(iidol.slate.ro?.us" <br />If any Survey Monuments are damaged during this permit installation, the Permittee will be held <br />liable and will be required to hire a Professional Land Surveyor, acceptable to CDOT, to re- <br />establish the Survey Monuments. All surveying costs are the responsibility of the Permittee. <br />The selected Contractor shall submit a Traffic Control Plan and Certificate of Insurance at least <br />ten (10) days prior to construction. Failure to do so may void this permit. CDOT Pennit <br />Number shall be noted on the Traffic Control Plan when submitted. <br />Revised 10/13/2010 7
