The Contractor shall utilize a spotter to assist in the visual inspection of all excavation work as it <br />progresses near existing CDOT Intelligent Transportation Systems fiber optic line conduits, <br />pull boxes and manholes. The Contractor shall provide a spotter to aid equipment operators when <br />construction activities are near marked or unmarked fiber lines. <br />The spotter shall observe all excavation work as it progresses to ensure that no damage occurs to <br />existing underground fiber lines. When the spotter has visual sight of the underground conduit, <br />the spotter shall notify the equipment operator of the proximity to the conduit and begin to guide <br />the excavation work. The spotter shall guide all excavation work around the conduit to ensure no <br />damage occurs. <br />If more than one area is being excavated the Contractor shall have a spotter at each location. <br />Spotters shall remain until all work is complete in the fiber optic line area and is secured and safe <br />in each location. All OSHA, CDOT and associated safety standards shall be followed at all times. <br />Uamuge to CUOT /Fiber Optic installations shall be paid by the contractor-dgent who causes <br />such damage. CDOT reserves the right of approval and inspection of all damage repairs. CUOT <br />may require complete replacement of fiber cable if damage location is less than 1000'tf from a <br />manhole or lateral connection to comply with C.'DOT specifications. <br />This is a condition of this permit and Failure to comply will cancel this permit. <br />FAILURE T17 C'OAiPLY 117171 ANY OF THE CONDITIONS STIPULATED IN THIS <br />PERMIT OR PROVISIONS WILL BE GROUNDS FOR REVOCATION OF THIS PERMIT. <br />NO PERMITTING 117L BE ALLOff'ED ON THIS N'ORti FOR ONE (1) CI LENDER IT.-AR <br />FR0,11 Rti VOC-I TION l!NI.I: V.V OT III.'Rif'B'E S'T..!TI.D. <br />TRAFFIC CONTROL <br />A Traffic Control Plan (TCP), prepared in accordance with the Manual of Uniform Traffic <br />Control Devices (MUTCD) and supplements, is required prior to the start of any work. <br />The Traffic Control Plan (TCP) submitted with this permit is considered the Minimum <br />Requirement in accordance with the Manual of Uniform Traffic Control Devices (MUTCD) <br />and supplements. The Department may require a more stringent application of Traffic Control as <br />outlined in the Colorado Department of Transportation Region 2 Lane Closure Strategy. This <br />can be accessed through the Department's external website, %yww.dot.state. co.us, under "Permits <br />and Documents" tab, then "Traffic Guidelines and Information" tab. <br />Traffic control is the responsibility of the Permittee and Applicant (if applicable), and shall <br />adhere to the MUTCD at all times. <br />Any time that flaggers are used or a traffic stoppage is needed an Authorization and Declaration <br />of Temporary Speed Limits application shall be submitted and approved before the permit is <br />released.
