MJN <br />Permit No: 2100172-U Mile Post: 427 +/- <br />Highway No.: 050 Patrol No.: 12 <br />Permit Issued To: JBS Five Rivers Cattle Feeding, LLC Permit Expiration Date: 12-31-11 <br />Contractor to be named before work begins <br />SPECIAL PROVISIONS FOR UTILITY AND OTHER PERMIT OPERATIONS <br />The Special Provisions are terms and conditions of this permit. <br />Effective May 2001 <br />A Pre Construction meeting including CDOT personnel shall be conducted for <br />any project over 1000' in State Highway ROW. <br />Projects which encompass 1000' or more of buried installation shall have an intended <br />placement or running line marked or staked out before a permit will be issued. <br />The CDOT Inspectors for this permit are Mark Nusskern, (719) 546-5433 and/or <br />Mike Watson, Rudy Marquez, (719) 336-9079. <br />This permit generally consists of the bored installation of 12" waterline and casing under the <br />roadway. This installation shall be greater than 60" under roadway surface and within a suitable <br />casing from toe of slope to toe of slope. <br />The permittee shall conform to all requirements set forth in the State Highway Utility <br />Accommodation Code. In issues of dispute the SHUAC provisions will prevail. Copies of this may be <br />accessed @ hlt??:/w?ti??.dot.state.co.uvutilit,_proLranv'. <br />Stormwater Management Plan/Erosion Control Supervisor is required regardless of the <br />size of the job/project or scope of work. <br />The Contractor/Permittee/Applicant Shall Perform The Following: <br />Designate A SWMP Administrator/Erosion Control Supervisor. Designate/identify the <br />individual(s) responsible for implementing, maintaining and revising SWMP, including the title <br />and contact information. The activities and responsibilities of the administrator shall address all <br />aspects of the projects SWMP, Best Management Practices (BMPs) For Stormwater Pollution <br />Prevention Phased BMP Implementation as the job/project progresses. <br />Any work in or along the SHROW shall require erosion control measures to control <br />sedimentation and water pollution caused by highway construction activities. <br />UNCC does NOT schedule locates for Colorado Department of Transportation Facilities or <br />other Government installations. It is the sole responsibility of the contractor to locate ALL <br />utilities. <br />For locates of CDOT-owned underground facilities, contact Dave Pierce at (719) 251-3596. <br />CDOT Statewide Fiber Optics Locates contact Dave Judy (303)-512-5830 <br />The Permittee is hereby advised that other utilities may exist within the proposed permit area. <br />Permittee shall implement any and all measures to protect any existing utilities from damage. <br />Revised 10/13/2010 1
