Rocky Mountain Wireline Service (RMWS) based out of Grand Junction, Colorado, <br />performed the secondary perforation operations prior to the P&A. <br />Daub & Associates, Inc. (D&A), based in Grand Junction, Colorado, engineered the P&A <br />and secured the contract. A geologist from D&A was on-site during all phases of the <br />abandonment operation, provided field coordination-supervision, and quality control of <br />the P&A activity. The D&A geologist performed the following duties: <br />• provided QA/QC of all operations <br />• maintained a chronological log of daily activities <br />• coordinated contractor operations <br />• provided cost tracking and reporting <br />Downhole Video Operations <br />On July 13, 2010 at 10:30 hrs, Jet West set up over the IA-5HR to run a video camera <br />downhole in an attempt to locate the casing cuts that were made in the 7.625 inch liner in <br />July of 2008. The camera was run from the surface to a depth of 2132 feet, at which <br />point the decision was made to stop the log due to extreme hole deviation. The attempted <br />casing cuts were identified at the depths of 1836 feet and 1840.5 feet. It was noted that <br />the casing cut did not pass all the way through the casing. <br />A second video log was run using a 3.5 inch sidescan camera on July 14, 2010 at 08:40 <br />hrs in an attempt to locate a casing cut at a depth of approximately 1756 feet. Water level <br />was noted at a depth of 545 feet, and at a depth of 1756.3 feet, scored casing was <br />identified. The Daub geologist noted that the casing cut did not appear to pass all the <br />way through the casing. <br />Bridge Plug and Perforation Operations--First Phase <br />On August 30, 2010 at 09:30 hrs, Jet West ran a 6.363 inch gauge ring into the hole to <br />identify any possible tight spots in the casing prior to running the CIBP. No casing issues <br />were identified, as the gauge ring was able to be run to a depth of 2110 feet, and the tool <br />was brought out of the hole slowly so as to locate the casing collars. At 11:00 hrs Jet <br />West ran in the hole with a 7.625 inch CIBP and Baker setting tool. The CIBP was <br />successfully set at the depths of 1912 feet to 1914 feet by 13:15 hrs. <br />At 13:45 hrs, Jet West ran in the hole with a 3.5 inch select-fire hollow steel carrier <br />perforation gun to perform the initial perforation operations on the 7.625 inch liner. Jet <br />West perforated two three-foot intervals with two 3/8 inch shots per foot with 120 degree <br />phasing. The first interval was between the depths of 1882 and 1885 feet, and a total of <br />six shots were made across this interval. The second perforated interval was between the <br />depths of 1902 and 1905 feet. A total of six shots were fired across this interval. Upon <br />pulling out of the hole, the water level was noted to have dropped approximately 100 feet <br />to a depth of 650 feet at this time. After confirming that all shots were fired (12 total), Jet <br />West rigged down and moved off location by 14:45 hrs on August 30, 2010. The <br />Daub & Associates, Inc.
