These include the East Pyeatt ponds, Johnson Gulch ponds,
and No Name ponds.
Most sediment ponds on the site were inspected, and the following maintenance issues were observed:
• Continual maintenance of No Name #2 is needed. In particular, the downstream bank requires armoring.
• Sediment level markers are missing for Ute Pond, Grouse #2, No Name #4R, No Name #5R, West Pyeatt
ponds. This problem should be rectified when possible.
• Oak Gulch Pond, East Pyeatt #1, and East Middle Flume are in need of sediment removal from the ponds.
This has been initiated for East Pyeatt #1 and East Middle Flume.
• Middle Pyeatt #3, Grouse #2, and West Horse Gulch Pond are in need of sediment removal from the
flumes.
West Horse bank has gullies that should be continually observed. If they get larger, they should be filled.
Burrow holes were observed on the East Flume embankment.
The asphalt rundown from Impoundment H has a small crack that requires repair.
A brush pile is downstream of Deer Pond and should be observed to prevent discharge of pollutants.
The inspection indicated no water quality problems, such as oil and grease or turbid water, at any of the pond
discharge points.
The Industrial Waste Pond was inspected. The boom is now functional. The outlet pipe is not visible.
GENERAL MINE PLAN COMPLIANCE:
Activities at K Ridge Pit, F Pit, and other areas were observed. The mine was found to be in general
compliance with the permit.
A potential issue is the storage of items (tires, cables, etc.) outside of the facility area.
OTHER (SPECIFY):
Number of Partial Inspection this Fiscal Year: 0
Number of Complete Inspections this Fiscal Year: 1
Page 3 of 11
