The previous property owners where Kimberley A. and Charles A. Reid. The consultant started <br />working on the maps and went to the Huerfano County Assessors office to get the Tax Map of <br />the property and the surrounding landowners names and addresses. Since that was the names on <br />the county assessors office had the map was prepared using those names, however later on it <br />was discovered that the new landowners where Len L. Camp and Roxic L. Camp. This change <br />was made on the maps, apparently I didn't proof the maps as well as 1 should have and the error <br />was made the map. This mistake will be corrected on a revised Reclamation Plan Maps attached <br />to this report. <br />6.4.12 Exhibit L- Reclamation Costs <br />12. The applicant will agree to the Division of Reclamation, Mining and Safety assessment of the <br />reclamation costs and will prepare the required financial warranty to reclaim the site using the <br />dollar amount of $ 61,000.00. <br />6.4.13 Exhibit M- other Permits and Licenses <br />13. The operator / applicant will commit and supply to the Office all of the necessary permits as <br />noted by the Division of Reclamation, Mining and Safety in the Permit Application Adequacy <br />Review letter dated April 23, 2010. The Huerfano County CUP 09-30 is being re-submitted and <br />high-lighted for clarity please see the attached document. The applicant will commit to <br />procuring the Colorado Air Pollution Control Division Fugitive Dust Permit and the Air <br />Pollution Emission Notice (APEN), water Quality Control Commission, Stromwater Discharge <br />permit and if needed an army Corp of Engineers 404 Permit. <br />6.4.18 Exhibit R - Proof of Filing with the Huerfano County Clerk and Recorder <br />14. Please see the Receipt from the Huerfano County Clerk and Recorder attached concerning the <br />revised application information as per the Division of Reclamation, Mining and Safety rules. <br />6.4.19 Exhibit S- Permanent Man-made Structures <br />15. The applicant is enclosing documentation from both the San Isabel Electric Association, Inc. <br />(SIEA), and the Hucrfano Board of County Commissioners (BOCC) concerning the Power <br />Line, and the County Road.
