Waste Containment and Disposal <br />NO CHANGE <br />Reclamation <br />All areas not part of our active Pilot Test - such as topsoil stockpiles, road edges, pad edges, and <br />stormwater control features - have been reclaimed following an interim reclamation procedure. <br />This interim reclamation procedure used a seed mix composed of only grass species which allows <br />control of broad-leaf weeds using selective herbicides. AMSO employs a company that is <br />certified to apply herbicides on BLM lands and that operates under a BLM Pesticide Use Permit <br />(PUP). <br />About one-third of the HB Pad and one-third of the MWP 2 Pad have been reclaimed following <br />the final reclamation guidelines and seed mix specified in AMSO's BLM Lease and NOI. <br />6 Proposed Disturbance (approximate) Describe the proposed drilling to be conducted, <br />including anticipated number of holes, diameter, depth, location, etc... <br />The following information is presented in accordance with the requirements set forth in the <br />NOI application Section III, 6.A., 6.B., and 6.C. <br />Modifications to the AMSO drilling program for the remaining wells to be completed are as <br />follow: <br />• Complete Tomography Wells TM-2 thru TM-6 following the completion details shown <br />below at the location shown in Figures A.2 and A.3 <br />• Complete T-1P, the Production Well, and T-1H, the Heater Well at the locations shown <br />in Figures A.2 and A.3, following the details shown in Figure A.4, and following the <br />completion details shown below. <br />Tomography Wells TM-2, TM-3, TM-5, TM-6 <br />• Drill a 10.625" from surface to a minimum of 80 feet. <br />• Run 7.625", 26.44, J-55 casing to 80 feet. <br />• Cement to surface with neat cement. <br />• Nipple up BOP and wait on cement a minimum of 4 hours. Pressure test BOP. <br />• Drill out through the surface conductor with a 6.25" diameter bit using mud or aerated <br />mud. Advance the drill hole to 2400 feet (TD). The Total Depth location must be within <br />a 2 ft. radius relative to the staked surface location. <br />• Run 4.5", 11.6#, J-55 casing to TD (2400 feet). <br />• Rig up for nitrogen aerated cement job. Cement casing to surface. <br />• Wait on Cement for 8 hours <br />• Drill out cement within the casing with 3.875" bit. Stop drilling at the casing shoe. <br />• Run Cement Bond Log after completion of all Tomography Wells. <br />6
