Denison Mines (USA) Corp. <br />Van 4 Mine - Ore Storage Pad <br />• 9. The geomembrane shall be kept free of debris, unnecessary tools and materials. In <br />general, the geomembrane area shall remain neat in appearance. <br />10. Equipment necessary to perform the installation shall have a scrap geomembrane sheet <br />placed underneath to protect the installed geomembrane from possible damage. <br />11. No welder or testing equipment shall be allowed to remain on top of the installed <br />geomembrane overnight. All equipment must be removed and stored away from the <br />installed geomembrane. <br />12. No fueling of INSTALLER's equipment will be allowed on top of the installed <br />geomembrane. No fuel containers shall be allowed on the geomembrane. <br />13. Any vehicle used prior to or after liner placement shall be first approved by the <br />ENGINEER. All vehicles are restricted from traveling on the liner material unless a <br />temporary access is constructed. <br />3.03 FIELD QUALITY CONTROL <br />A. Visual Inspection <br />1. Following placement of the geomembrane, the ENGINEER and the INSTALLER will <br />visually inspect the liner for any kind of defect or damage during shipment and/or <br />installation. All repairs will be conducted as described below. <br />• B. Repair Procedures <br />1. All geomembrane panels and seams shall be examined by the ENGINEER for defects, <br />holes, blisters, undispersed raw materials, and any sign of contamination by foreign <br />matter. The geomembrane surface shall be clean at the time of examination. Each suspect <br />location shall be repaired and all repairs shall be nondestructively tested. <br />2. Damaged geomembrane shall be removed and replaced with acceptable geomembrane if <br />damage cannot be repaired to the satisfaction of the ENGINEER. <br />3. Any portion of the geomembrane, or any portion of a seam exhibiting a flaw shall be <br />repaired as follows: <br />a. Geomembrane patches shall be used for holes over 1/8 of an inch in diameter, <br />tears, and contamination by foreign matter. Patches shall be constructed of the <br />same geomembrane, and will be joined to the panel using adhesive or chemical <br />fusion welding where possible. <br />b. Geomembrane patches or caps shall extend at least 6 inches beyond the edge of <br />the defect or failed seam area, and all corners of material to be patched. The <br />corners of the patch shall be rounded. <br />C. Wrinkles <br />1. Large wrinkles that remain in the sheet as result of temperature expansion or uneven <br />surface preparation may need removal as determined by the ENGINEER in consideration <br />JOB NO. 64986. 10.1 6
