<br />(Page 3) <br />MINE'ID # OR PROSPECTING ID #: M-197810.1 <br />INSPECTION DATE: 2/1410 INSPECTOR=S INITIALS: MAC' <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified <br />during this inspection. The problems should be corrected by the dates given, or they will become possible <br />violations. The possible violations should be corrected by the dates given to reduce their severity when considered <br />by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />1. The west boundary of the Gaede Pit was not delineated. Rule 3.1.12(2) requires the DATE <br />boundaries of the affected area to be marked by monuments or other markers that are <br />clearly visible and adequate to delineate such boundaries. <br />Corrective actions are to: The Operator shall mark the western boundary of the Gaede Pit <br />with monuments or other markers. The boundary must be delineated on or before March 19, <br />2010 and proof of compliance must be submitted by the same date. Proof of compliance <br />may be in the form of photographs. <br />2. Erosion of the reclaimed pit slope along the southern boundary has created rills. The <br />Operator has failed to stabilize all surface areas of the affected land to effectively control <br />erosion per the requirements of Rule 3.1.6(3) <br />Corrective actions are to: The Operator shall repair the rills that have formed on the southern <br />pit slope. In addition, the Operator shall implement erosion control measures such as straw <br />bales, silt fences, or revegetation of the slopes to effectively control the erosion. The Operator <br />shall repair the rills on or before April 16, 2010 and proof of compliance must be submitted by <br />the same date.
