STORMWATER MANAGED T PLAN 2/20/09 <br />Maryland Creek Ranch <br />7. EXTERNAL REPORTING REQUIREMENTS <br />7.A. ANNUAL REPORT <br />As required by the CDPHE-WQCD in the CDPS General Permit for this mine site, Everist will <br />submit an annual stormwater report, covering January 1 through December 31 of each year, on <br />the overall compliance with the SWMP. Everist will submit the report on or before February 15 <br />of each year, unless otherwise directed by the CDPHE-WQCD. Everist will submit the CDPHE- <br />WQCD form entitled: Stormwater Annual Report - Sand and Gravel. A copy of the annual <br />report form is included in the appendix for reference. <br />Information recorded from the bi-annual Comprehensive Facility Inspections will be used to <br />complete the annual report to the CDPHE-WQCD. <br />7.13. SAMPLING AND MONITORING REPORT <br />The permit for this site allows Process Water Discharges (mine de-watering). Representative <br />samples will be analyzed as per the parameters in Part I.B. of the CDPS General Permit. A <br />copy of the General Permit is attached for complete details of sampling and parameters to be <br />monitored. They include flow, pH, total suspended solids, oil and grease, total dissolved solids <br />and total phosphorus. <br />The CDPHE-WQCD requires data on process water discharges to be submitted on a quarterly <br />basis. This sampling and monitoring data will be submitted on a Discharge Monitoring Report <br />(DMR) supplied by the CDPHE-WQCD on or before the dates listed below. <br />Quarter Samples Taken Report Due Before <br />Q1 Jan-Feb-March April 28 <br />Q2 Apr-May-Jun July 28 <br />Q3 Jul-Aug-Sep October 28 <br />Q4 Oct-Nov-Dec January 28 <br />9
