August 2002 -G- 003 -2203 <br />shaft remediation. A mix design was proposed by Ames using, Type IM <br />cement. The concrete supplier, Transit Mix of Colorado Springs, <br />• Colorado,, furnished sulfate resistance testing data for Holnam Type 1/II <br />cement, which demonstrated the proposed cement met the sulfate resistant <br />requirement of ASTM C150 -97A for Type V cement. Therefore, Golder <br />approved the use of Holnam Type 1/11 cement in the mix design. <br />February 2002: Golder recommended remediation of UG -545 (Squaw <br />Mountain Tunnel) by backfilling the mined void with a flowable, <br />cement -based fill. <br />► February 2002: Golder approved a modification to the Phase IV Diversion <br />Channel wing -wall alignment. The wing -wall alignment was modified to <br />tie the wall directly into competent bedrock, thus forming one side of the <br />wall with bedrock. <br />► April 2002: Golder approved removal of the solution collection piping <br />within the Phase IVa Area. The piping was deemed unnecessary for <br />solution recovery by CC &V. Golder determined no impact to the design <br />by removal of these pipes. <br />► April 2002: Golder accepted a variance in the Fine Shaft Backfill (FSB) <br />gradation for the specific quantity of material tested. The variance in the <br />FSB gradation was considered insignificant by Golder based on experience <br />with similar projects. <br />► July 2002: Golder approved a modification to the three 42 -inch CSP for <br />the Phase IVa Haul Road crossing. Golder extended the three 42 -inch <br />CSP approximately 25 feet to accommodate additional cover material on <br />the south end. <br />1.5 Record Survey Documentation <br />Ames Construction of Aurora, Colorado provided the Record Drawings for the activities <br />associated with the Phase IVa Area. A Colorado Registered Land Surveyor prepared the <br />drawings. The Record Drawings are presented in Appendix A. The Surveyor's certificates are <br />presented in Appendix F.
