August 2002 -5- 003 -2203 <br />1.3.1 Daily Construction Summary Reports <br />• <br />Golder prepared daily construction summary reports as well as photographic documentation <br />(Appendix $) throughout the project to document the key elements of construction and the <br />CQA activities that occurred. On -site Golder CQA personnel, the Certifying Engineer and the <br />Project Engineers discussed construction activities on a .regular basis. In addition, field <br />personnel prepared daily observation reports. Golder submitted these daily reports and a daily <br />summary report to CC &V. Daily reports are not included in this report'but were available for <br />Colorado Division of Minerals and Geology (DMG) review during the course of the project. <br />1.3.2 Weekly Construction Summary Reports <br />Golder prepared weekly construction summary reports for Phase IVa, b, and c areas, which <br />were submitted to CC &V and to the DMG. The weekly reports during the period of February <br />2001 through August 2002 are presented in Appendix D. <br />1.4 Project Specifications <br />All work was performed in accordance with the design plans and specifications in Amendment <br />No. 8 to Mining Permit M -1980 -244 and subsequent revisions as approved by DMG. <br />Construction drawings are presented in the Drawings section of this report and project <br />specifications are presented in Appendix E. <br />If construction and/.or specification modifications were warranted due to site - specific <br />conditions, review and ,approval by the DMG was requested. No changes were implemented <br />without prior approval ,from the DMG. A summary of items that were addressed during <br />construction are summarized below as follows: <br />December 2001: Golder approved an alternate cement type. The <br />specifications for the Phase IV construction stipulate the use of Type V <br />(sulfate resistant) cement for the concrete plugs and cemented rockf ll for <br />I .
