AM-04 Incompleteness <br />1 October 2009 <br />Pg. 2 <br />Rule 6.4.3, Exhibit C - Pre-mining and Mining Plan Map(s) of Affected Lands: Please <br />update as needed or indicate where in previous documentation the information may be found. <br />Rule 6.4.4, Exhibit D - Mining Plan: Please describe any modifications to the mining plan that <br />are proposed in Amendment AM-04 over that submitted previously. <br />Rule 6.4.5, Exhibit E - Reclamation Plan: The replacement of the slurry wall with a clay liner <br />represents a substantial change to the existing reclamation plan. Please provide the following <br />information about the clay liner: <br />• Source of liner material <br />• Engineering design <br />• Installation plan, methods, and schedule <br />• Testing/certification plan <br />• An engineering stability analysis for the proposed final reclaimed slopes of the reservoir <br />embankments, as required under Rule 6.5. <br />Rule 6.4.6, Exhibit F - Reclamation Plan Map: Please provide a revised reclamation plan <br />map depicting the clay liner in a fully installed configuration. <br />Rule 6.4.7, Exhibit G - Water Information: Please provide any updated water information, <br />including plans for pumping and disposal of pit water during liner installation, and any other <br />impacts to the prevailing hydrologic balance that may be new in this Amendment. <br />Rule 6.4.8, Exhibit H - Wildlife Information: Please update as needed or indicate where in <br />previous documentation the information may be found. <br />Rule 6.4.9, Exhibit I - Soils Information: Please update as needed or indicate where in <br />previous documentation the information may be found. <br />Rule 6.4.10, Exhibit J - Vegetation Information: Please update as needed or'indicate where in <br />previous documentation the information may be found. <br />Rule 6.4.11, Exhibit K- Climate: Please update as needed or indicate wherein previous <br />documentation the information may be found. <br />Rule 6.4.12, Exhibit L - Reclamation Costs: The replacement of the slurry wall with a clay <br />liner represents a substantial change to the reclamation plan. Therefore, a new estimate of <br />reclamation costs must be provided. Please include the estimated cost of the liner, including the <br />cost of materials, labor, and overhead. <br />Rule 6.4.13, Exhibit M - Other Permits and Licenses: Please update as needed to include, at <br />a minimum, certification of the clay liner by the Office of the State Engineer. <br />Rule 6.4.14, Exhibit N - Source of Legal Right to Enter: No update needed.
