AM-04 Incompleteness <br />1 October 2009 <br />Pg. 3 <br />Rule 6.4.15, Exhibit O - Owner(s) of Record of Affected Land (Surface Area), and Owners <br />of Substance to be Mined: Please update as needed or indicate where in previous <br />documentation the information may be found. <br />Rule 6.4.16, Exhibit P - Municipalities Within Two Miles: No update needed. <br />Rule 6.4.17, Exhibit O - Proof of Mailing of Notices to County Commissioners and Soil <br />Conservation District: As explained earlier under General Application Procedures, please <br />provide adequate proof of the required notices. <br />Rule 6.4.18, Exhibit R - Proof of Filing with County Cleark and Recorder: As explained <br />earlier under General Application Procedures, please provide adequate proof of filing. <br />Rule 6.4.19, Exhibit S - Permanent Man-Made Structures: Please update as needed or <br />indicate where in previous documentation the information may be found <br />Rule 6.5 - Geotechnical Stability Exhibit: Please provide engineering stability analyses for the <br />proposed final reclaimed reservoir embankments. Information for slope stability analyses may <br />include, but would not be limited to, slope angles and configurations, compaction and density, <br />physical characteristics of earthen materials, pore pressure information, slope height, post- <br />placement use of site, and information on structures or facilities that could be adversely affected <br />by slope failure. <br />Additional Considerations: Be advised that, after the application has been designated as <br />complete and filed, applicant must comply with the public notice requirements described in <br />Rules 1.6.2(1)(d) through Rule 1.6.2(1)(g), and Rule 1.6.5. <br />Applicant has 60 days from the date of this letter (30 November 2009) to provide the <br />missing information per Rule 1.4.1(8) or the application may be denied.
