INJECTION WELL-SPECIFIC REQUIREMENTS <br />Well Name: 10H-I <br />EPA Well ID Number: C030358-08414 <br />Prior to Commencing Injection Operations, the permittee shall submit the following information: <br />1. Completed Well Completion Report (EPA Form No. 7520-9) and schematic diagram. <br />2. Summary of daily drilling activities <br />3. Depth to the top of each formation or unit encountered and any water quality or other <br />pertinent data that is collected. <br />4. Gamma Ray Log (GR), Caliper Log (CAL), Spontaneous Potential (SP)/Resistivity Log <br />5. Cement Bond Log (CBL) or Cement Evaluation Log <br />6. A successful Part I (internal) Mechanical Integrity test (MIT) <br />7. Temperature Log <br />8. AOR well l OH-C : CBL <br />The permittee shall includea-ppromiate narrative interpretations by a knowledgeable log analyst <br />along with pertinent logs. Once these records are received and approved by EPA, the Director <br />will provide written authorization to inject. <br />Underground Sources of Drinking Water (USDWs) <br />Starting from the ground surface, USDWs in the Area Permit are: a perched or unconfined <br />discontinuous low permeability sandstone aquifer approximately 400 feet below surface, the 16 <br />feet thick A-Groove Aquifer, and the 290 feet thick B.-Groove Aquifer. <br />Aquifer Exemption is not required in the Dissolution Surface Zone because the TDS of water . <br />produced from the proposed injection well is greater than 10,000 mg/l. The L-5 zone <br />immediately overlying the Dissolution Surface has been exempted. <br />Approved Injection Zone <br />Injection is approved below the Dissolution Surface, in the L-5 zone which comprises the Boies <br />Bed (L-5A), L-513, L-5C, and L-5E zones. <br />Confining Zone <br />The R-6 zone aquitard which lies directly above the L-5 zone, has been designated as the <br />confining interval. It is approximately 177 feet of marlstone, oil shale, and leached oil shale <br />between the depths of approximately 1600 ft and 1775 ft. <br />Maximum Allowable Injection Pressure (MAIP) <br />The MAIP is 300 psis as measured at the surface. <br />Operational Fluid Level Limitation <br />At all times the rate of injection shall never exceed the rate of recovery pumping (verified by <br />weekly average flows) plus the amount of fluid needed to replace the volume of produced <br />nahcolite. <br />Authorization for Additional Well: UIC Area Permit C030358-00000 <br />Well: 10H-1 EPA Well ID: C030358-08414 Page 2 of 5
