Final UIC Area Permit No.C030358-00000, effective December <br />4, 2003, authorizes injection for the purpose of solution mining in the Natural Soda, Inc. lease. <br />On October 28, 2008, Natural Soda, Inc. notified the Director concerning the following <br />additional injection well: <br />Well Name: 1OH-I <br />EPA Well ID Number: C030358-08414 <br />Location: 253' FNL & 447' FWL <br />SW SW Sec. 25 T1S-R98W <br />Rio Blanco County, CO <br />Pursuant to 40 CFR §144.33, Area UIC Permit No. C030358-00000 authorizes the <br />permittee to construct and operate, convert, or plug and abandon additional injection well(s) <br />within the area permit. This well was determined to satisfy additional well criteria required by <br />the permit. <br />This well is subject to all provisions of UIC Area Permit No. C030358-00000, as <br />modified and as specified in the Injection Well-Specific 'Requirements detailed below. This <br />Authorization shall expire one year after the Effective Date unless the permittee has converted <br />the well to injection or submits a written request to'extend this Authorization prior to the <br />expiration date. <br />This Authorization is effective upon signature. <br />SUN 44 2009' <br />Date: <br />Stephen S.
