COR-040000 Stormwater Management Plan <br />3) Reporti?rgAddress <br />A signed copy of the report form(s) will be submitted to the following address: <br />Colorado Department of Public Health and Environment <br />Water Quality Control Division <br />WQCD-P-B2 <br />4300 Cherry Creek Drive South <br />Denver, Colorado 80246-1530 <br />b) Monitoring (COR-040000, §I.E.2): <br />Sampling and testing of stormwater for specific parameters is not required on a routine basis <br />under this permit. <br />c) Signatory requirements (COR-040000, §I.E.5): <br />All reports and applications submitted to the CDPHE and/or EPA will be signed and certified <br />as follows for accuracy by an authorized Denison representative responsible for the overall <br />operation of the facility from which the discharge described in the form originates: <br />"I certify under penalty of law that this document and all attachments were prepared <br />under my direction or supervision in accordance with a system designed to assure that <br />qualified personnel properly gather and evaluate the information submitted. Based on my <br />inquiry of the person or persons who manage the system, or those persons directly <br />responsible for gathering the information, the information submitted is, to the best of my <br />knowledge and belief, true, accurate, and complete. I am aware that there are significant <br />penalties for submitting false information, including the possibility of fine and <br />imprisonment for knowing violations." <br />d) Recordkeeping and internal reporting procedures (COR-040000, §I.E.6): <br />Denison will retain a copy of the SWMP for the duration of the permit. <br />Incidents such as spills or other discharges, along with other information describing the quality <br />and quantity of stormwater discharges, will be included in the records. Inspections and <br />maintenance activities will be documented and recorded. Denison will retain copies of such <br />records for at least three (3) years from the date generated. All reports to the CDPHE and any <br />relevant correspondence will be retained for at least five (5) years from the date generated. <br /> <br />Van 4 Shaft SWMP 23 Denison Mines (USA) Corp.
