COR-040000 Stormwater Management Plan <br />8. REPORTING REQUIREMENTS (COR-040000, §I.E): <br />a) Annual Report (COR-040000, §I.E.1)s <br />I? A?znualReporlforActiyet?li?iingOperalions <br />Denison will submit an Annual Report, covering January 1 through December 31 of each <br />year, on the overall compliance with this SWMP. The Annual Report is due to the CDPHE <br />on or before February 15 of each year. The exact due date for Denison's first Annual Report <br />will be listed in the permit certification. The Annual Report will contain, at a minimum: <br />• Permitted facility name, address, phone number, and permit certification number. <br />• A report on the facility's overall compliance with the SWMP. <br />• Changes made in the individual items of the SWMP, and any proposed changes. <br />• A summary of each comprehensive stormwater facility inspection made, including date, <br />findings, and action taken. <br />• Results and interpretation of any stormwater monitoring performed. <br />• The report will be signed and certified for accuracy by Denison, including the <br />certification language outlined below. <br />• As required in CDPS Permit No. COR04 0248, the first annual report form, including the <br />Discharge Monitoring Report section, should be filled out as indicated and submitted to eke <br />first fepeFt may inelude less than twelve fnenths of infefmatien, Unless etheFwise indieated <br />en the e°~"` eatien CDPHE office by February 15, 2010. The GDPHE may Fequest <br />needed. <br />2) Complra1zce Reporlfor Inactive 01nAag Operations <br />If the Van 4 Shaft becomes inactive, Denison will submit a report on the compliance with the <br />SWMP. The report will be due to CDPHE on or before February 15, 2011 and cover the <br />period of time between the coverage by this version of the general permit and December 31, <br />2010. The report will contain, at a minimum: <br />• Permitted facility name, address, phone number, and permit certification number. <br />• A report on the facility's overall compliance with the SWMP. <br />• A summary of the annual inspection reports, including date, findings, and action taken, <br />or the triennial certification by a Professional Engineer (based on one or more <br />inspections) that the facility is in compliance with the permit (see Section 5.b).
