COR-040000 Stormwater Management Plan <br />' 2) Any repairs or maintenance needs identified by the inspection will be completed <br />immediately. Based on the results of the inspection, if revisions to the description of <br />the potential pollutant sources and the pollution prevention and control measures are <br />needed, the plan will be revised as appropriate as soon as practicable after the <br />inspection. Revised control measures will be implemented in a timely manner, but in <br />no case more than 90 calendar days after the inspection. <br />3) A report summarizing the scope of the inspection, personnel making the inspection, <br />the date(s) of the inspection, significant observations relating to the implementation <br />of this SWMP, and actions taken in accordance with the previous paragraph will be <br />made and retained for at least three years after the date of the inspection. Significant <br />observations include such things as the locations of discharges of pollutants from the <br />site; locations of previously unidentified sources of pollutants; locations of BMPs <br />needing maintenance or repair; locations of failed BMPs that need replacement; and <br />locations where additional BMPs are needed. The report will also document any <br />incidents of noncompliance observed. This record will be summarized in the Annual <br />or Compliance Report (see Section 8). <br />d) Monitoring (COR-040000, §I.D.6) <br />• (12/12/2008) Amendment: <br />During the period beginning November 17, 2008 and lasting through September 30, 2011, <br />the Van 4 Shaft Monogram Mesa facility is subject to the monitoring requirements issued in <br />the permit certification at each discharge point of stormwater discharge from the facility. <br />Representative Discharge: When a facility has two or more outfalls that, based on a <br />consideration of features, materials and activities within the area drained by the outfall, with <br />substantially identical effluents, Denison may test the effluent of one such outfall and report <br />that the quantitative data also applies to the substantially identical outfalls. In addition, for <br />each outfall that is representative, an estimate of the size of the drainage area (in square feet) <br />and an estimate of the runoff coefficient of the drainage area (e.g. low, medium, or high) will <br />be provided. <br />Samples will be taken twice per year, at least one month apart, unless otherwise directed by <br />the Division. Below is a summary of the sampling requirements for the Van 4 Shaft facility <br />(also see the sampling guidance document provided with permit certification located in <br />Appendix A): <br />I) AdaVionalfrrformaliorr <br />The following additional information is required for each sampling event: <br />• The date and duration (in hours) of the storm event(s) sampled; <br />Van 4 Shaft SWMP 17 Denison Mines (USA) Corp.
