COR-040000 Stormwater Management Plan
' 2) Any repairs or maintenance needs identified by the inspection will be completed
immediately. Based on the results of the inspection, if revisions to the description of
the potential pollutant sources and the pollution prevention and control measures are
needed, the plan will be revised as appropriate as soon as practicable after the
inspection. Revised control measures will be implemented in a timely manner, but in
no case more than 90 calendar days after the inspection.
3) A report summarizing the scope of the inspection, personnel making the inspection,
the date(s) of the inspection, significant observations relating to the implementation
of this SWMP, and actions taken in accordance with the previous paragraph will be
made and retained for at least three years after the date of the inspection. Significant
observations include such things as the locations of discharges of pollutants from the
site; locations of previously unidentified sources of pollutants; locations of BMPs
needing maintenance or repair; locations of failed BMPs that need replacement; and
locations where additional BMPs are needed. The report will also document any
incidents of noncompliance observed. This record will be summarized in the Annual
or Compliance Report (see Section 8).
d) Monitoring (COR-040000, §I.D.6)
• (12/12/2008) Amendment:
During the period beginning November 17, 2008 and lasting through September 30, 2011,
the Van 4 Shaft Monogram Mesa facility is subject to the monitoring requirements issued in
the permit certification at each discharge point of stormwater discharge from the facility.
Representative Discharge: When a facility has two or more outfalls that, based on a
consideration of features, materials and activities within the area drained by the outfall, with
substantially identical effluents, Denison may test the effluent of one such outfall and report
that the quantitative data also applies to the substantially identical outfalls. In addition, for
each outfall that is representative, an estimate of the size of the drainage area (in square feet)
and an estimate of the runoff coefficient of the drainage area (e.g. low, medium, or high) will
be provided.
Samples will be taken twice per year, at least one month apart, unless otherwise directed by
the Division. Below is a summary of the sampling requirements for the Van 4 Shaft facility
(also see the sampling guidance document provided with permit certification located in
Appendix A):
I) AdaVionalfrrformaliorr
The following additional information is required for each sampling event:
• The date and duration (in hours) of the storm event(s) sampled;
Van 4 Shaft SWMP 17 Denison Mines (USA) Corp.
